Position Summary

The Memory Matters Program Specialist is responsible for the planning, coordination and evaluation of the Memory Matters Program provided through the Aging & Disability Resource Center of Walworth County.

SCHEDULE

Days: To be Determined
Work Hours: 7:30 am to 3:00 pm
Hours Per Shift: 7
Shifts Per Pay Period: 2

Essential Duties and Responsibilities

This job description reflects management's assignment of essential functions: it does not prescribe or restrict the tasks that may be assigned.

Receive referrals from Aging and Disability Resource Center Dementia Care Specialist and determine eligibility for the Memory Matters Program.

Meet with potential participants to screen appropriateness for participation in the Program.

Provide information to participant and caregiver related to the Memory Matters Program.

Plan, facilitate and evaluate programming beneficial to individuals with Alzheimer's disease and other dementias.

Determine equipment and supplies needed for programming and coordinate with the ADRC Dementia Care Specialist to secure needed items.

Maintain a daily log of attendance and participation notes for each participant and report all data as determined by the Walworth County Health and Human Services.

Communicate with participant and caregiver areas of concern or support needed for participant's success in the Program.

Provide transitional support to consumer as needed and refer to Aging and Disability Resource staff for additional services if appropriate.

Complete access and abuse neglect reports according to State statute and forward to APS team for screening review.

Collaborate with other programs and individuals across department to ensure coordination of care.

Participate in trainings to update knowledge and skills.

Ensure compliance with program standards and maintain necessary documentation as required by Walworth County Department of Health & Human Services, Department of Health Services and state and federal statutes.

Demonstrate a commitment to county safety and risk management efforts.

Qualifications

Education and/or Experience

Bachelor's or Associate's degree in social work or a related field and at least one year of relevant prior experience required. Experience working with the aging and disabled populations preferred. Knowledge of Trauma Informed Care Principles is essential.

Interactions & Communication

Provides specialized information and/or recommendations to others regarding an area of expertise. This may include direct care agencies, hospitals, community agencies, and/or the general public.

Certificates, Licenses, Registrations and other Requirements

Valid driver's license.

Proof of minimum liability auto insurance.

Must be able to provide a reliable personal vehicle to be used for business related travel as needed.

Decision Making

This position has authority to make decisions that are consistent with policies and precedents; supervision and managerial direction is available as requested. These decisions directly impact performance in the job and/or the work unit within the department and information is provided with additional research and observations which contributes to the decision making process.

In relation to established procedures, protocols and policies of the County, the complexity of this job is low and challenges are relatively minimal.
