Job Description:

The MDS Clinical Coordinator is responsible for participating in the coordination of patient admissions, serving as a liaison between the patient, family members and financial organizations to ensure that services needed meet and are authorized to so resident care can be provided. Ensures that financial authorizations are obtained timely, that PSARR's, TAR's and ARF's are obtained timely and accurately. Assists in preparation and implementation of a discharge plan for each resident including oversight of process, obtaining needed authorization, DME's and MD Order. Provides ongoing education to the staff in Sub Acute as well as ongoing CEU program for the Certified Nursing Assistants. Provides support to the Unit by ensuring accurate and timely completion and submission of the resident assessment instrument, including the MDS, to ensure compliance with regulations. Completes accurate plans of care for all residents.Southern California Hospital at Culver City has been taking care of generations of Culver City and west LA residents. Now at 420 beds, SCH-CC is a general acute care hospital that utilizes many of the latest medical treatments and technologies. The hospital offers a wide range of inpatient and outpatient acute care services, including an orthopedic center, cardiovascular services, acute rehabilitation, sub-acute care, psychiatric care and chemical dependency programs. Additionally, SCH-CC operates a 24-hour emergency services center, which serves as a paramedic receiving station and is staffed by board-certified emergency physicians and nurse specialists. \n

• Active, current, and unencumbered Licensure as a Vocational Nurse in the State of California
• Minimum Two (2) years experience or equivalent care coordination experience.
• AHA Basic Life Support
• Ability to read, write and communicate effectively in English
• Ability to maintain flexibility and composure in a constantly changing environment

Preferred Qualifications:
• MDS Coordinator Certification
• Director of Staff Development (DSD) Certification

Pay Rate: Min - $39.25 | Max - $52.73


  • Ensures Medi-Cal Treatment Authorization Request's (TAR) / CalOptima's Authorization Request Forms (ARF), are completed timely. Works with Central Business Office on financial requests and ensures timely and accurate completion. Ensures Re-Authorizations are completed timely and accurately and electronically documented in the designated location(s). Works with Insurance Authorization for PPO's and Managed Care Organizations as needed. Responsible for calculating the PDPM Category the patient falls into (based on data collected), documenting the projected PDPM Category and maintaining daily communication with Nurse Manager/Program Director on resident's status and PDPM categories. Ensures timely transition of all PDPM information to the appropriate state agency.

  • Coordinates the Interdisciplinary Comprehensive Care Plan as part of MDS process and completes the Comprehensive Assessment tools (RAI - Resident Assessment Instrument). Ensures that the documentation in the residents' medical record supports the MDS and the interdisciplinary plan of care. Signs the MDS and ensures all other therapists' signatures are present. Actively Participates in Interdisciplinary Team Meetings and Initial family meetings. Participates in Family Forums as needed. Coordinates and assists with completion and submission of accurate, timely, inter-departmental patient assessments.

  • Facilitates training and learning activities through development of education programs and materials for sub-acute staff. Delivers educational programs through formal and informal means, makes effective and efficient use of time and resources, manages learning environment, and assists learners to identify learning needs. Gives direction and feedback to participants as needed. Ensures educational regulatory requirements are maintained. Ensures that staff implement Infection Prevention and control practices per hospital policy.

  • Begins the interdisciplinary discharge process timely and includes members of the interdisciplinary team (including providing process oversight, Authorizations and MD Order). Provides timely documentation in the medical record of the Discharge Planning Discussions (monthly). Assists with the discharge planning process including obtaining necessary authorizations and arranging needed durable medical equipment (DME).

  • Ensures Pre-Admission Screening and Resident Review (PASRR) are completed timely and maintained in medical record. Ensures appropriate admitting authorization is received and electronically documented in the designated location(s).
