Overview:
The Primary Care Nurse Practitioner, along with a multi-disciplinary clinical team, provides acute and primary care services to patients residing in skilled nursing and other community-based facilities. Conducts patient assessment, physical exam, ordering of laboratory and radiology tests along with interpretation and reporting of the results. Documents the delivery of care in the appropriate EMR(s). Provides patient and family education, counseling, and referral to other health care providers as appropriate. Performs related responsibilities as required.

RESPONSIBILITIES
Deliver high quality patient care onsite at partnering facilities or via a secure telehealth platform
Collaborate with facility staff, primary care team(s) and specialists, as appropriate
Assists with assessment, diagnosis, treatment, and management of acute and chronic health conditions
Orders and interprets laboratory and radiology tests within the scope of professional practice
Prescribes medications, including controlled substances, to the extent delegated and licensed
Orders treatments and durable medical equipment as indicated
Performs other therapeutic or corrective measures as indicated, including triage of acute and urgent care issues
Informs and consults PCP and/or specialists of the services provided and collaborates with other health care providers with the assistance of the population health clinical coordinator
Prepares and maintains accurate patient records via the EMR(s) to support sound medical practice and communication with other health care providers
Complies with applicable laws and regulations with respect to collaborative agreements
Documents appropriately for the reimbursement of services rendered
Provides training and continuing education for colleagues and/or other staff
Assists in the development of clinical practice guidelines/standards in support of evidence-based, quality patient care
Adheres to the practice of confidentiality regarding patients, families, staff, and the Organization
May provide cross-coverage for Palliative/Hospice programs as needed
QUALIFICATIONS
Active State License
NPI #
Desire to be in a patient-centered, healthcare setting
Excellent bedside manner with solid customer-oriented presentation skills
Capable of multi-tasking in a fast-paced environment, where you are often moving from station to station
Ability to working autonomously and adapt to changing environments
Talent for cultivating a positive and energetic team atmosphere
Strong computer proficiency
Flexibility to travel by car to community events within 1 hour from home location
Must have a valid Driver's License with a clear driving record
ANCC or AANP board certified required
Ability to lift up to 50 lbs. on a consistent, daily basis
Actively or recently participated in service to Medicare patients and Medicaid patients (preferred)
