Join our team of competent and compassionate professionals as a Rehabilitation & Support Specialist in the PACT (Program for Assertive Community Treatment) program at Behavioral Health Network!
Your experience, knowledge and great skills will support the PACT program with the following:
Providing direct and indirect services to individuals served by the program.
Monitoring individuals' health and well-being.
Supporting the growth, independence, and safety of individuals served.
Helping individuals maintain and enhance their social support network through opportunities to meet new friends, build stronger relationships with family members and guardians, attend religious services and groups, and foster engagement in other community groups, organizations, and relationships.
Planning and providing opportunities for community activities.
Completing initial assessments, treatment plans and reviews, financial management plans, and assessments to assigned caseload.
Providing safe and timely transportation in own vehicle, as needed.
Assisting with all aspects of medication treatment and administration.
Who are we?
The Program for Assertive Community Treatment (PACT) program is a highly integrated outreach and 24/7 residential program that helps individuals meet their goals and reduce hospitalizations. PACT provides clinical wraparound support in the community and in our group residential program to perpetuate recovery and wellness for the individuals served. The PACT team is forensic-focused and designed to reduce arrests and incarcerations.

What are we looking for in a Rehabilitation & Support Specialist?
Commitment to the empowerment and growth, human rights, and community integration of individuals served.
At least six months of experience in human services (required).
High School Diploma or GED equivalent (required). Bachelor's Degree in related field (preferred)
