Your Role:



As a Nurse Practitioner, you'll play a vital role in enhancing the health and well-being of our members. Your responsibilities will include:

• 
  In-Home Assessments: Conducting comprehensive health assessments in patients' homes.
• 
  Medical Reviews: Reviewing medical history, medications, and functional status.
• 
  Patient Education: Providing education on health management, medication adherence, and disease prevention.
• 
  Care Gap Identification: Identifying and addressing care gaps to improve patient outcomes.
• 
  Collaboration: Collaborating with healthcare teams to ensure seamless care coordination.

Qualifications:

• 
  Licensure: Valid Nurse Practitioner license in the state of [State]
• 
  Experience: Previous experience in primary care, geriatrics, or home health is preferred.
• 
  Clinical Skills: Strong clinical assessment and diagnostic skills.
