Provide personal care services under the direction of the Office Manager. The Care Tech/CNA is assigned to specific clients by the Office Manager, Administrator or other appropriate professional and performs services for clients as necessary to maintain their personal comfort in accordance to their Service Plan.
Qualifications/ Requirements
Successful completion of a formal training program and/or a written skills test and competency evaluation.
Be at least eighteen (18) years of age and a documented citizen of the United States
Must be high school graduate or have received equivalent degree
Minimum of one (1) year work experience in a supervised setting preferably health care facility.
Must follow Strick COVID-19 guidelines
Ability to read, write, and follow a written Service plan of Care
Good verbal, written, and interpersonal skills
Licensed driver with automobile insured in accordance with stat and/or GWCC requirements and in good repair/driving record.
Must be self-directed and work with Minimal Supervision
Demonstrate dependability, tact, patience, and the ability to follow instructions.
Demonstrate good interpersonal communications skills.
Have experience in cooking, cleaning, laundry, and shopping.
Essential functions/Area of Accountability
Assists client with short term/long term services in accordance to their Service Plan,
Performs personal care activities, including but not limited to:
a. Bathing/Hair Care
b. Dressing
c. Skin/fingernail care
d. Personal & Oral hygiene
e. Shaving (Electric only)
f. Assist w/feeding
g. Assisting w/transfers
h. Medication reminders
3. Assists with meal planning, purchase of food, and food preparation.
4. Assists clients to live in clean safe environment:
a. Cleans bathrooms and kitchen, including counters & refrigerator
b. Personal errands, per client and GWCC request
c. Laundry
d. Vacuuming and dusting
e. Changing bed linens
5. Attends to the client's request promptly.
6. Provides companionship, socialization, and housekeeping services.
7. Immediately reports any observed or reported changes in the client's condition and/or needs to the Office Manager or Administrator. Reports any signs of abuse or neglect to GWCC administrative staff.
8. Documents care provided and completes the forms required for the client's records daily. Completes the appropriate records to document care given and pertinent observations. Submits documentation to administrative staff per GWCC requirements at least weekly.
9. Promotes personal safety and a safe environment for clients by observing infection control practices, following GWCC guidelines, and reporting unsafe situations to the Office Manager/Administrator. Also, knows and follows infection control policies including proper hand washing/hand sanitizing/ glove and mask wearing techniques.
10. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff zoom meetings, and team meetings, as requested.
11. Treats clients & family with respect and always maintains confidentiality in all aspects of the job.
12. Attends in-service program to meet compliance requirements. Identifies learning needs to GWCC Office Manager without fear of reprisal.
13. Performs other related duties and responsibilities as deemed necessary.
14. Accepts and fulfills assignments with GWCC and exercises judgment in accepting assignments without fear of reprisal.
