JOB SUMMARY

The Healthy Transitions Community Liaison is responsible for building relationships with existing referral sources regionally and identifying new sources for the sole purpose of increasing the number of persons served VOACC’s behavioral health services throughout our regions.   Further, engaging in community activities to educate on VOACC therapeutic model.  Finally, in collaboration with program leadership, will create and participate/lead direct consumer engagement and attractions that identifies, links, aligns, maintains, and grows the reputation of VOACC’s behavioral health services as an agency of choice.

RESPONSIBILITIES:

Consumer Attraction Engagement


  • Develops and implements both short-term and long-term consumer attraction plans with measurable goals and objectives aligned with organizational goals.

  • Identifies and develops new referral agencies and manages existing referral agency relationships by addressing concerns and participating in “think tanks” to become a resource for agencies that are referring persons served for care.

  • Manages a systems approach to document and track all referring agencies, number of referrals and feedback from referring agencies to address concerns and continue to increase the pipeline.

  • Develops and stays current with cultivation and referral strategies, particularly in the area of person served and retention initiatives of referring agencies which celebrates diversity teams and persons served.

  •  Achieve monthly & quarterly revenue targets (TBD)

  • Achieve & maintain a minimum of 15 active referral sources.

  • Achieve a minimum of (10) quarterly Unique Admissions (i.e. Day Rehab, Yo Cree En Mi, MAT, SUD, MAI, Medical Resp)

  • Achieve a minimum of 15 quarterly new referrals.

  •  Successfully conduct 20 quarterly professional presentations to area referral sources/community partners

  • Assist in expanding Service Amenity usage to 70% minimum.

Person Centered Branding


  • Coordinates with marketing team to publicize programs that are user friendly and celebrates diversity.

  • Partners with the marketing team to create and update web site content to promote VOACC therapeutic model and modes of receiving new referring agencies that supports diversity and communities.  

Organizational Responsibilities


  • Participates in committee meetings, staff meetings and other activities and/or meetings contributing to the effective, efficient operation of the referral process for various programs in all regions.

  • Adheres to organizational expectations as outlined in Volunteers of America’s Chesapeake and Carolina’s policies and procedures.

  • Provide technical assistance with software applications as needed.

  • Demonstrates Core Values on a daily basis through personal behaviors and interactions with staff from the ministry and diversity equity and inclusion communities.  

EFFECT ON END RESULT:


  • Achieves goals aimed at engaging referring agency partners in meaningful opportunities in support of the agency’s work and methods of care. 

  • Responsible for developing and executing consumer attraction engagement strategies for advancing the vision and initiatives set by the various programs and organizational vision 2025.  

REQUIREMENTS


  • Bachelor’s degree in Marketing, Business Administration or related field with a specialty in business to business marketing/engagement. Master’s degree in related field a plus not required.   

  • Minimum of three (3) to five (5) years of related experience working in the non-profit

  • Experience conducting consumer engagement attraction (i.e. day/evening fairs/civic meetings etc.) activities and community partnership for client referrals for services.   

  • Experience in pharmaceutical sales, or general consumer engagement sales knowledge preferred.  

  • Experienced working as a case worker, counselor or providing services to persons served within the human services field requirement.   

  • Experience in taking a diverse, equitable and inclusive approach to working with persons served.

  • Demonstrates initiative and adaptability when working with technology and new software

  • Excellent ability to communicate effectively in sensitive and/or demanding situations. 

  • Demonstrates the ability to professionally represent Volunteers of America Chesapeake and Carolina’s effectively and interact with multi-cultural and diverse communities.

  • Strong problem solving and execution skills.

  • Strong PC skills in a Windows environment (i.e. Word, Excel, Power point and email)

  • Ability to use independent judgment and address conflict and find resolutions.

  • Excellent interpersonal, written, and oral communication skills.

  • Bilingual in any language a plus.

  • The ability to safely operate a motor vehicle to transport oneself, consumers, and program supplies as necessary.

  • The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment.

  • Operating office equipment requiring continuous or repetitive hand/arm movements.

  • The ability to remain in a sitting position for extended periods of time
