ORGANIZATION BACKGROUND
Housing for Health (HFH) is a program office within Health Services Administration, a division under the Los Angeles County Department of Health Services (DHS). HFH was created and put into implementation in support of the Los Angeles County Homeless Initiative recommendations in response to and in support of the County’s effort to address and combat homelessness in the communities residing within Los Angeles County. Our organization follows a hybrid work structure where employees work both remotely and from the office, as needed.
PROJECT BACKGROUND
The Department of Health Services (DHS) Housing for Health (HFH) program is a cohesive team of clinicians, community health workers, program administrators, and other professionals who collaborate with multiple County departments, community-based service providers, cities, and other partners to provide housing and critical services to People Experiencing Homelessness (PEH) and those who have previously experienced homelessness. HFH works to meet the needs of people with complex health and behavioral health conditions. In response to the homelessness crisis in Los Angeles County, the Los Angeles County Departments of Health Services (DHS), Mental Health (DMH), and Public Health (DPH) are coming together to form regional, field-based, multidisciplinary teams called Interim Housing Assessment and Action Teams (IHAAT) to address gaps in behavioral health and physical health services among functionally disabled People Experiencing Homelessness (PEH) who are living in interim housing (IH) and connect them to stabilizing services (such as in home caregiving, higher level of care, or enhanced care management/community support). The IHAAT teams will comprise of both medical and behavioral health staff and will prioritize and expedite social and housing navigation services in all eight (8) Los Angeles County Service Planning Areas (SPAs). This position is a one-year funded position.
As part of the initiative for the Interim Housing Assessment and Action Teams (IHAAT), also known as the Interim Housing Outreach Program (IHOP), Housing for Health will sponsor the Activities of Daily Living (ADL) program to increase access and support for PEH with ADL needs in interim housing (IH). These teams will perform comprehensive assessments of high-risk PEH in IH settings, refer them to supportive services, increase their function and safety within their current environments, and assist them with transitioning them to higher levels of care, as appropriate. The Interim Housing Outreach Program (IHOP) teams will serve clients in shelters, motels, and tiny homes located in all eight Service Panning Areas (SPAs) in Los Angeles County. DHS-sponsored IHOP staff, hired by the DHS Housing for Health (HFH) program, will include advanced practice providers, nurses, emergency medical technicians, and occupational therapists.
The Staff Assistant, IHOP will assist and support all IHOP operations.

DESIRABLE QUALIFICATIONS
Two years of experience in either (1) a staff capacity analyzing and making recommendations for the solution of problems of organization, procedure, program, budget or personnel, or (2) a highly responsible secretarial capacity, assistant, or coordinator or (3) a responsible supervisory clerical capacity.
SCHEDULING
Specific schedule to be determined up to 40 hours per week. Proposed field-based teams operating hours are Monday through Friday from 7:00 a.m. to 6:30 p.m.; set scheduled will be addressed at time of onboarding but ideally 8am – 5pm with some possibility for variation based on field-based teams’ hours. Hybrid model consisting of office-based work, remote work, and minimal field work. Be available to travel throughout LA County’s Service Plan Areas 1-8, if necessary.
