Overview

Family Medicine at Richmond Clinic is a primary care Family Medicine clinic and Federally Qualified Health Center (FQHC). The patient population includes pregnant people, babies, children, adolescent, adults, and geriatric. The patients are generally moderately stable to moderately unstable, with complex medical conditions and SDOH barriers that require medical and nursing interventions. These include, but not excluding: oral and injectable medications, foot care, telephone and in-clinic triage, care coordination, health education, complex wound care, pain management, and behavioral and/or crisis management.

Nurses perform patient visits, telephone triage and advice, and manage an in-basket workload in EPIC where pts. are reviewed for Care Management, Care Coordination, follow up for discharge planning, and triage/advise for medical conditions. Care Coordination and Disease Management are a high priority aspect of the job. Nurses work in pods within a care team of Providers, Pharmacists, and Behavioral Health clinicians.

Visit types offered at Family Medicine at Richmond are: In-person, virtual, and telephone.

Department Specific Requirements:

  • External Candidates: Vacation, paid or unpaid leave requests up to 8-weeks from the start date are not permitted. All other vacation, paid or unpaid leave requests are subject to compliance with the Collective Bargaining Agreement between ONA and OHSU.
  • Internal Nurse Candidates: All vacation, paid or unpaid leave requests are subject to compliance with the Collective Bargaining Agreement between ONA and OHSU. Pre-scheduled vacations or leave plans must be disclosed prior to consideration for hire to determine the start
  • 2 years or more, within the last three years, experience working in direct patient care.

Department Specific Preferences (competencies, certifications, student placements, quality matching, etc.):

  • Within the last 3 years, experience in working an ambulatory care setting including telephone triage.
  • National certification.


Responsibilities

The qualifications for the position are the minimum requirements to be eligible for a pre-screen and/or interview. The level of required experience and expertise for each posted position is influenced by the current amount of knowledge and skills of the other nurses in the unit/departments

  • Baccalaureate degree in Nursing for external hires; Baccalaureate degree preferred to internal
  • Baccalaureate degree in Nursing awarded from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA) by 30 days before the start

  • Current unencumbered Oregon RN license by 1 week before the start date, unencumbered Washington RN license within 90 days of hire
  • Current BLS certification awarded by the American Heart Association (AHA)

Working Conditions:

  • Ability to lift, bend, stoop, reach, carry, push, pull and walk throughout the work Able to lift 35 lbs. independently. Complies with the safe patient mobilization policy when lifting or positioning dependent patients, including using mechanical lifts and coordinating the assistance of other personnel.
  • The work environment carries a risk of exposure to human tissue/fluids.
