Job Summary: JOB SUMMARY: a brief description of the job. The PAD (Peripheral Arterial Disease) Cardiac Rehab Coordinator is responsible for the planning, development, implementation, and ongoing oversight of the Supervised Exercise Training (PAD) Rehab program. The Coordinator is responsible for coordinating overall standards of population appropriate patient care, financial performance, and coordination of department activities with the operations of other departments to provide quality services. The Coordinator is responsible for the implementation and administration of all PAD education for the appropriate patients across the system. Performance of this job will also require a close working relationship with the Medical staff, referring physicians as well as community outreach. The PAD Coordinator will be a highly competent and energetic professional with passion and expertise to promote the PAD Rehab program's impact, sustainability, and visibility. Core Responsibilities and Essential Functions: Department Operations and Program Development *Responsible for developing, implementing, and on-going oversite of a system-wide PAD Rehab (Supervised Exercise Therapy) program *Collaborate with system cardiac rehab leadership to implement/integrate into existing Cardiac Rehab programs. *Work with Cardiologists, Vascular Surgeons and other clinical leaders to create guideline-directed, evidence based clinical protocols *Create patient educational material specifically targeting the needs of patients with PAD *Accountable for oversight of data collection and outcomes reporting. *Conduct outreach and foster relationships with key stakeholders: Vascular Surgeons, Cardiologists, PCP and the community. *Develop and provide education for referring providers in collaboration with MD Clinical Champion. *Participate in screening events within Wellstar and with key strategic partners *Regular communication with managers and site leaders regarding program challenges and anticipated needs. *Ensure that CMS and other regulatory requirements for the SET program are being met *Develop staff competencies specific to the treatment of patients exercising with PAD. *Arrange educational opportunities for the teams to ensure they are informed of necessary concepts that are essential to provide high quality care to these patients. Patient Care *Assesses patient risk factors that are modifiable and provides strategies to aid patient in successful therapeutic lifestyle changes. *Participates in initial assessment and orientation of new participants. Assists in development of plan of care with other staff members. *Develops and adjusts individual exercise prescription in accordance with daily patient responses to exercise according to ACSM and AACVPR guidelines. *Supervises exercise session, monitors participants for inappropriate responses to exercise. *Responsible for blood pressure management, observation of telemetry responses and blood glucose measurement per department policy. *Documents daily on patient responses in ScottCare and Epic. Teaching/Education *Assesses patient's knowledge of cardiovascular disease, nutrition, risk factors, and develops educational plan to meet individual needs. *Leads patient education in 1:1 and/or group setting. Team Responsibilities *Participates in emergency training sessions and assists as directed in emergency situations. *Supports and assists in development of performance and process improvement initiatives designed to promote the highest quality patient care. *Maintain flexibility to respond to the changing needs of the team. *Contributes to cohesive group relations and communicate effectively with team members about individual patient findings or concerns about program issues. Required Minimum Education:


  • Bachelors Health Promotion or Bachelors Exercise Science or Masters Other-Preferred


Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated.

  • Basic Life Support or BLS - Instructor

  • Advanced Cardiac Life Support within 180 Days or ACLS - Provisional within 180 Days or ACLS - Instructor within 180 Days


Additional License(s) and Certification(s):
Required Minimum Experience: Minimum 2 years working in Cardiac Rehab related field Required or Required Required Minimum Skills: Strong leadership skills including the ability to coordinate with multiple departments within organization. High Solid business development skills, proven track record in achieving performance goals and knowledge of healthcare and cardiac rehab marketing. Medium Strong leadership skills and the ability to supervise staff. Medium Strong organizational skills. Strong communication skills with the ability to act and resolve issues independently. High Professional communication skills, including ability to address large groups. High Ability to facilitate change. Must be able to interpret research findings and apply them to management of Cardiac Rehabilitation. High A strong desire to work with a comprehensive health care team. High Financial skills to include but not limited to interpretation of reports regarding department statistics, expenses, revenues; ability to prepare and educate subordinates regarding flexible operating budget and a contingent capital budget.
