Job Description

Housing for Health (HFH) at NYC Health + Hospitals (H+H) recognizes that the chronic health issues of unhoused individuals cannot be treated without stable housing. This initiative seeks to improve the health and wellbeing of New Yorkers by focusing on four strategic areas: housing navigation services, medical respite beds, affordable housing on hospital property, and social service support for patients in permanent housing.

Under the direction of the Manager, Respite Operations, the Project Manager will be responsible for supporting the Housing for Health team to manage expanding medical respite operations as well as permanent housing placements for patients experiencing homelessness.

General tasks and responsibilities include but not limited to:

Coordination:
  • Perform intake for patients experiencing homelessness referred to Housing for Health with one-on-one support. This includes reviewing H+H facility referrals to confirm patient's eligibility; liaising with referral sources for additional patient information and consent; and matching patient to respite vendors and housing navigation partners.
  • Coordinate with H+H facilities personnel, nonprofit entities, government agencies, and housing providers to support patients' housing placement.
  • Keep detailed notes of case conference discussion, tracking progress on follow-up
  • Support escalations for barriers to housing and discharge planning, including by liaising with partner staff, other members of Housing for Health team, and track housing escalation cases
  • Program and project management for other housing, medical respite, and real estate initiatives as assigned.

Data Analysis:
  • Support day to day data management, including analysis and reporting to monitor contract performance, goals, and quality improvement efforts; matching patients to housing opportunities; identification of patient housing barriers; and data clean up.
  • Merge and parse data sets supplied by all Housing for Health programs efficiently and be comfortable transforming dirty big data into actionable knowledge.
  • Communicateanalyses clearly to lay audiences and use analysis to drive progress towards H+H housing goals with facility partners, external entities, and the Housing Location and Placement Services and medical respite vendors.
  • Utilize data analysis to drive coordination and policy creation with internal stakeholders, contracted vendors, government agencies, and housing providers.
  • Assists in designing evaluative measures including data analysis and procedures by which program efficiency and costs effectiveness can be audited and assist in evaluation audits.
  • Assist with tracking and data analysis for ad-hoc projects as assigned.

Miscellaneous:
  • Perform research and other information gathering activities and prepare these materials for input into grant proposals, reports, and/or meetings and conferences.
  • Support the Housing for Health teams needs across all programs as requested.
  • Manage and assist with special projects as requested by Assistant Vice President and Senior Directors

Minimum Qualifications

  1. A baccalaureate degree in engineering, architecture, landscape architecture, business administration or public administration from an accredited college or university; and one year of full-time satisfactory experience in project management work, such as planning, administrating, managing, coordinating or expediting for engineering and/or architectural and/or landscape architectural projects; or
  2. A four-year high school diploma or its educational equivalent and five years of experience as described in "1" above; or
  3. A four-year high school diploma or its educational equivalent plus any combination of the experience and/or college education as described in "1" above to make up the equivalent of five years of education and experience. One year of credit will be given for each 30 semester credits of college education leading to a baccalaureate degree from an accredited college or university in engineering, architecture, landscape architecture, business administration or in public administration.

Special Note:
  1. Experience which is primarily of a design nature is not acceptable towards meeting the minimum requirements.

  • New York City residency (Brooklyn, Bronx, Manhattan, Queens or Staten Island) preferred.
  • Interest in housing and homelessness in a healthcare setting
  • Experience with data analysis
  • Excellent written and oral communication skills.
