Job Description:

General Purpose
The Program Director oversees the care coordination program and clinically supportive services of Solvista Health.

Essential Duties/Responsibilities
Responsible for administrative and clinical supervision of the care coordination and case management programs. Responsible for providing care coordination and case management services in conjunction with clinical services. Responsible for adherence to system-wide quality assurance monitoring program. Will attend and represent the organization at Health Colorado Care Coordination meetings. Responsible for program deliverables as an accountable partner. Responsible for annual care coordination audit. Establishes, implements and evaluates policy, procedure and program content. Prepares, reviews and administers program budgets. Selects, orients, trains, counsels, disciplines, and directs all program staff. Evaluates and monitors program needs, identifies unmet needs and revises services as needed. Provides organization, direction and staffing according to the scope of services needed. Organizes and coordinates system-wide projects. Plans and consults with others as necessary. Problem-solves issues with other services, as indicated. Conducts in-service training. Participates in and conducts meetings, as necessary. Serve as an information resource to clinical team. Participate in job-related training sessions and seminars. Attend and/or facilitate required staff meetings.Perform other duties as assigned.

Job Qualifications
Knowledge, Skills, and Ability:
Possesses excellent organizational skills. Knowledge of specific medical/psychiatric illnesses, procedures and treatments Possesses excellent interpersonal and negotiation skills necessary to work within a team Possesses cultural sensitivity and demonstrated competency in age specific behaviors Ability to work effectively as a member of a multi-disciplinary team. Knowledge of community agencies/resources. Ability to analyze data, draw conclusions, prioritize and make recommendations. Ability to multitask under stressful situations. Ability to work within a Windows operating system and related programs. Ability to communicate clearly and effectively verbally and in writing. Ability to communicate effectively with a diverse population. Ability to work competently and consistently through sleep interruption or deprivation. Ability to prepare and maintain accurate records Ability to interpret rules and regulations, read and understand client records. Ability to maintain objectivity and confidentiality.

Education or Formal Training:
Bachelor's degree, consistent with the nature of the program that is being supervised.

Experience:
Minimum of five (5) years of Supervisory Experience.
Minimum of three (3) years of experience supervising coordination, case management, or related program(s).

Working Environment:
This job operates in a fast-paced, professional office environment and routinely requires the use of standard office equipment such as computers, phones, photocopiers. The position may require travel to conferences, meetings and branch locations on a regular or intermittent basis. Work may involve moderate exposure to unusual elements, such as extreme temperatures, dirt, dust, fumes, smoke, unpleasant odors, and/or loud noises. In the health center environment, there is potential for contact with blood-borne pathogens and communicable diseases, as well as potential for contact with dissatisfied or abusive individuals. There could be interaction with persons who are mentally ill, disabled, elderly and emotionally upset.

Physical Activities:
These are representative of those which must be met to successfully perform the essential functions of this job.
This is a largely sedentary role but can involve standing or sitting for extended periods of time, bending at the waist, and using hands and fingers to handle and file papers or operate assigned equipment. While performing the duties of this job, the employee is regularly required to talk or hear. Specific vision abilities required by this job include close vision, distance vision, color vision, and ability to adjust focus. Employee may also have to lift 10 to 25 lbs. unassisted.

Conditions of Employment:
Annual TB, federally required drug screening, and Influenza vaccination or compliance with policy and procedure.
Required possession of a valid state driver's license.
Successful candidate must submit to and pass, post-offer drug screen.
