Education: Bachelor's degree in social services or a related field.


Experience: At least two years' experience in case management, job development or crisis intervention with youth. A Master's degree in a social services related field may be used to substitute for one year of experience. Experience in community building, event coordination and public speaking is preferred.


Summary: The Transition and Financial Specialist is responsible for using sound professional judgement and best practices to ensure quality services are provided to the service population especially in crises situations while maintaining compliance with established state and federal standards.

Essential Functions:
1. Assesses personal needs of PAL clients and help to obtain resources to meet those needs by providing case management services.
2. Advocates with human services, health, and court and criminal justice systems to resolve issues facing clients.
3. Assists youth in crisis by providing de-escalation for situation and developing plan with youth to move out of state of crisis.
4. Develops detailed Individual Service Plans for each client and meet with them regularly to assess progress and update the plan.
5. Maintains timely and accurate file documentation and data entry that meets the requirements of both the PAL and TWC programs.
6. Follows all DFPS guidelines and policies concerning PAL Transition and Financial Services and TWC services.


Measurable Deliverables:
1. Meets all federal and state regulatory guidelines and standards that are applicable to this position.
2. Participates/leads PAL program weekly, monthly events and activities.
3. Work evenings, weekends and holidays as needed or requested by position supervisor.
4. Implement BCFS safety protocols including evacuating with children and other staff in case of an emergency.
5. Maintains confidentiality in all areas of the service population and program operations.
6. Maintains BCFS HHS professional and ethical standards of conduct outlined in the BCFS employee handbook including demonstrating respect for agency staff, the service population, and community members and complying with the required dress code at all times.
7. Field calls, emails, visitors on daily basis.
8. Maintains front lobby with proper daily items.
9. Attend all required trainings as applicable.
10. Maintains operational office equipment and supplies at all times


Credentials, Skills and Abilities:
1. Maintain current licenses and certificates required for the position including but not limited to:
• First Aid • CPR • Other
2. Pass a pre-employment drug screen and random drug screens and maintain emotional control and professional composure at all times.
3. Provide proof or work eligibility status upon request.
4. Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints needed if applicant has lived out of state within the past 5 years)
5. Demonstrate the ability to:
a. Respond sensitively and competently to the service population's cultural and
socio-economic characteristics.
b. Communicate effectively in writing and verbally in English and Spanish.
c. Work in a fast-paced environment.
d. Maintain computer literacy required to meet the responsibilities of the position.
e. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team.
f. Travel as needed.
g. Manage time effectively and efficiently.
6. Bilingual preferred (Spanish)
