Position Title: Care Management Support Coordinator II


Location: Durham, NC 27703 (Remote)




Anticipated Start Date: ASAP




Duration: 6+ months (Potential to Extend)




Job Type: Full Time/ W2




Pay Range: $22.00/hr on W2




Shift Timings: 8:00am EST - 5:00pm EST




Job Description: Care Management Support Coordinator II




Position Purpose: Supports administrative care management activities including performing outreach, answering inbound calls, and scheduling services. Serves as a point of contact to members, providers, and staff to resolve issues and documents member records in accordance with current state and regulatory guidelines.



Education/Experience:



  • Requires a High School diploma or GED.

  • Requires 1 - 2 years of related experience.

  • Provides outreach to members via phone to support with care plan next steps, community or health plan resources, questions or concerns related to scheduling and ongoing education for both the member and provider throughout care/service.

  • Provides support to members to connect them to other health plan and community resources to ensure they are receiving high-quality customer care/service.

  • May apply working knowledge of assigned health plan(s) activities and resources.

  • Serves as the front-line support on various member and/or provider inquiries, requests, or concerns which may include explaining care plan procedures, and protocols.

  • Supports member onboarding and day-to-day administrative duties including sending out welcome letters, related correspondence, and program educational materials to assist in the facilitation of a successful member/provider relationship.

  • Documents and maintains non-clinical member records to ensure standards of practice and policies are in accordance with state and regulatory requirements and provide to providers as needed.

  • Knowledge of existing benefits and resources locally and make referrals to address Social Determinants of Health (SDOH) needs.

  • Performs other duties as assigned.


Day-to-Day Responsibilities:



  • Assist in activities related to the medical and psychological aspects of utilization and coordinated care.

  • Conduct outreach attempts via telephone to assigned members daily.

  • Maintain and update an Excel spreadsheet with member assignments.

  • Maintain consistent communication with supervisor.

  • Document all interactions in Virtual Health or similar system with each phone call.


Performance Expectations/Metrics:



  • Make a high volume of outbound calls (80-120 calls/day).

  • Utilize medical terminology appropriately.

  • Maintain a reliable home internet connection with a dedicated workspace (HIPAA compliant).

  • Complete assignments for 600 members monthly (approximately 30 new members daily).

  • Achieve 100% compliance with assigned member tasks.

  • Maintain an engagement rate of 80% or higher monthly.

  • Achieve a minimum audit score of 90%.

  • No time off permitted in the first 90 days of employment unless pre-planned during the interview process.


First Day Expectations:



  • Meet with IT department to ensure all equipment and systems are operational.

  • Complete required trainings assigned by supervisor, spanning the entire first week.

  • Begin training specific to the job role once initial trainings are completed.


Previous Job Titles/Backgrounds:



  • Any role providing customer service in a medical field setting.


Internal/External Interfaces:



  • Interact with members, providers, and internal staff regularly to facilitate care coordination and resolve issues.


Required Skills/Experience:



  • Basic knowledge of Microsoft Office.

  • Experience in customer service, preferably in a healthcare setting.

  • Strong communication skills.


Preferred Skills/Experience:



  • 2-3 years in a managed care setting, medical office, or facility with medical administration duties.

  • Knowledge of medical terminology.

  • Strong time management and organizational skills.


Education Requirement:



  • High school diploma or equivalent.
