Responsibilities include integrating elements of patient management, examination, evaluation, diagnosis, prognosis, interventions, and documentation to maximize a patient's functional outcomes. Supervision of a PTA is th required.
Candidate Qualifications:
Graduate of APTA accredited institution with entry-level degree
Licensed and registered by the State of New York.
Experience in a skilled rehab setting is preferable.

Responsibilities include integrating elements of patient management, examination, evaluation, diagnosis, prognosis, interventions, and documentation to maximize a patient's functional outcomes. The occupational therapist will work with an interdisciplinary team to best meet our residents' needs. Supervision of a COTA is required.
Candidate Qualifications:
Graduate of an accredited institution with entry-level degree
Licensed and registered by the State of New York.
Experience in a skilled rehab setting is preferable.

Wells Nursing Home is seeking a speech therapist who will provide direct patient care,.. under the direction and prescription of a physician. Responsibilities include evaluation, diagnosis, prognosis, incorporation of interventions, and documentation of services to treat speech, language, social and cognitive communication, and swallowing disorders in adults on a per diem basis to maximize a patient's functional outcomes. The speech therapist will work with an interdisciplinary team to best meet our residents' needs.

Licensed and registered by the State of New York.
Experience in a skilled rehab setting is preferable.
All three positions will be contracts and $60.00 hr.
