Job Description:

KEY JOB RESPONSIBILITIES: Provides skilled occupational therapy service on an intermittent basis to patients in their homes under the supervision of the Occupational Therapist.

Additionally, the Occupational Therapist Assistant will:


  • Under the direction of the Occupational Therapist (OT) provides patient care services which have been delegated by the Occupational Therapist.

  • Under the direction of the OT assists in evaluation and development of the rehabilitative plan of care and in periodic re-evaluation as required.

  • Participates in the preparation of clinical and daily progress notes. All notes are reviewed by the OT and submitted weekly. Also prepares periodic summary reports.

  • Together with the OT, instructs the patient and family, assists in teaching them exercise programs.

  • Guides and instructs the patient in prescribed therapeutic self-care and creative activities that are directed toward improving independence and physical and mental functioning and assists the patient in the use of self-help devices.

  • Identifies possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and reports same to the OT.

  • Observes, and records activities and findings in the clinical record and reports to the OT and the physician the patient's reaction to treatment and any changes in the patient's condition.

  • Consults with OT regarding the outcome of home visits and reports physical findings in written form.

  • Under the supervision of the OT, assists in evaluation and development of the rehabilitative plan of care and in periodic re-evaluation as indicated.

  • Instructs other health team personnel including, as appropriate, Home Health Aides, as well as family members with respect to certain phases of occupational therapy, so that they may work with the patient.

  • Writes daily progress notes on patients, and submits same weekly, and prepares periodic summary reports as indicated.

  • Attends rehabilitation department staff meetings and case conferences.

  • Instructs patient in care and use of wheelchairs, braces, splints, and prosthetic and orthotic devices.

  • Participates with staff, patients, and physicians in discharge activities and assists the Occupational Therapist in completing the Occupational Therapy Discharge Summary.

  • Confirms, on a weekly basis, the scheduling of visits with the Supervisor/Director of Nursing in order to coordinate Agency visits.

  • Assumes responsibility for self-development by continually striving to improve his/her health care knowledge through educational programs, attendance at workshops and conferences, active participation in professional and related organizations and individual research and reading.

  • Assumes other duties deemed appropriate and necessary by the OT.

  • The ability to drive and the ability to access patients' homes which may not be routinely wheelchair accessible are required.

  • Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.

  • On occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 pounds.

  • Must be able to effectively communicate in English.

  • Help optimize the capital structure and support strategic initiatives.

  • Maintains the agency's mission, philosophy and core values.

  • Ensures compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.

  • Ensures compliance with policies and procedures regarding infection prevention, control, standard precautions, and infection identification reporting.

  • The clinical record shall contain notes for each service provided. A clinical note will be completed for each visit made to a patient's home (including private residence, assisted living facility, group homes or skilled nursing facility) within 48 hours of the visit.

  • Always maintains patient confidentiality, including all HIPAA regulations.

PROFESSIONAL EXPERIENCE/QUALIFICATIONS:

Industry experience


  • Must have at least one year of Home Health Care.

  • Comprehensive knowledge of general therapy theory and practice and able to work independently and with the Occupational Therapist.

Regulatory Requirements


  • Licensed or registered in the State of Michigan.

  • Must pass a criminal background check & MVR check.

  • Completed health statement acknowledging ability to perform the duties of the position.

  • Valid state driver's license.

  • Proof of current automobile insurance.

  • Current CPR card; On-line CPR certification accepted; Hands-on CPR certification preferred.

  • TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).

OTA Home Health Expertise


  • Must be computer literate and able maintain simple records in English.

  • Promotes patient/family education using various verbal and written communication techniques that take into account the patient's/family's cultural, ethnic and/or personal needs or preferences.

  • Collaborates with the patient/family to learn appropriate self-care techniques.

  • Submits documentation in a timely manner per Agency policy.

  • Knowledgeable of current federal, state and agency regulations concerning patient care, includes incident management and reporting.

  • Maintains regulatory requirements. Including complying with Legal Authority: Act 519, Public Acts of 1982; MCL 400.11, which states: All persons employed, licensed, registered, or certified to provide health care, must report suspicion or reasonable cause to believe that physical, mental, or sexual abuse, neglect, or exploitation of an adult has occurred.

  • Participates in agency's other mandated activities, i.e., in-services, Quality Assurance and Performance Improvement (QAPI); staff meetings.

EDUCATION:

  • Must be a graduate of an approved school giving a course of not less than two years for Occupational Therapy Assistants.
