Job Description:
Location: San Francisco, CA
Salary: $150,000.00 USD Hourly - $170,000.00 USD Hourly
Description: Our client is currently seeking a NP or PA - San Francisco
*This is a full-time, direct-hire role with a great salary and benefits
This job will have the following responsibilities:
- Conduct initial and annual face-to-face patient visits in home and around the community
- complete patient Health Risk Assessments
- Update and reevaluate the year-to-end diagnoses following care
- Identify diagnoses and develop the proper treatment
- Collaborate with HEDIS/STARS
- Complete EMR Documentation with quality
- Conduct patient referrals
- Other duties as assigned
Qualifications & Requirements:
- Active California State License as an NP or PA
- Computer Literacy/EMR experience
- 1 year of clinical experience
- Valid Drivers License
- BLS and ACLS
- DEA number (within 6 months of hire)
- NPI Number
