This position is responsible for providing effective mental health and substance abuse interventions to assigned clients (individuals and families), coordinating individual cases with primary care physicians, referrers, insurance carriers or other agencies, completing and maintaining documentation and records. Represents North Shore Medical Center in a professional manner and adheres to all North Shore Medical Center policies and guidelines and state and federal regulations and program requirements.
Meets with clients to assess their needs; provides complete diagnosis; in collaboration with clients determines manageable problem focus and treatment priorities. (Clients may include: families, adults, and/or elders.)
Presents timely evaluations and treatment plans to multi-disciplinary team for review and utilization management. Implements treatment directives developed at team meeting.
Interacts effectively with each client. Works with clients and, when appropriate, their support systems to produce behavioral, cognitive, and/or emotional changes that meet treatment goals. Evaluates client progress and revises treatment plans as needed.
Works collaboratively with payors, referrers, primary care physicians, and community agencies in providing effective treatment to clients.
Completes all necessary documentation within required time parameters for each client. For example: assessments and treatment plans, insurance authorizations, problem-oriented progress notes, and treatment summaries.
Qualifications
Masters Degree required.
LICENSURE: Must have license as a Mental Health provider or be in process of obtaining licensure. LCSW, LICSW, LMHC
