Job Summary:

The Program Manager manages either a state-licensed Adult Family Home, an independent living home, or a community support program for people with disabilities. This position includes approximately 20 scheduled hours of direct care and approximately 20 hours of administration each week. A rotating weekend on-call obligation is also a part of the Program Manager's schedule. The Program Manager is responsible for ensuring the safety and well-being of the members under his or her supervision. The Program Manager is also responsible for the supervision and training of the staff working with the members. This position necessitates ongoing quality communication with executive management, colleagues, staff, members, members’ support teams, and others in the community.

Essential Duties:

  • Treat all members and employees with respect and dignity.
  • Maintain and expand knowledge and understanding of existing and proposed federal, state, and local regulations relevant to the provision of services to individuals with disabilities. Uphold members’ rights and educate employees on those rights.
  • Advocate for the best interest of each member. Develop and implement care plans for each member which take into account their rights, interests, abilities, personal goals and preferences.
  • Create and implement Individual Support Plans (ISPs) for each of the members living in the Adult Family Home. Regularly review and update to ensure they are current and accurate. Provide copies of ISPs to employees to review at least twice per year.
  • Maintain and organize all necessary documentation for the members in the program, the program’s vehicle, and the home, in accordance with DHS regulation and ILC, LLC policy. Submit required documentation, such as house finance reports, in a timely manner to appropriate persons.
  • Develop meaningful, outcome-based goals for each member. Develop and implement plans to help each member achieve their goal(s) during the defined time period. Educate employees on members’ goals, and coach employees on appropriate methods for supporting member goals.
  • Oversee the medical care of each member. Schedule and attend medical appointments for each member. When the Program Manager’s attendance is not required at a medical appointment, assign a suitable employee to attend and/or arrange for transportation to and from the appointment.
  • Attend and participate in the weekly supervisor meeting. Attend additional supervisor meetings and/or training as scheduled as the CEO.
  • Participate in the weekend on-call rotation. Submit pertinent information and schedule updates to the assigned on-call supervisor in a timely manner each weekend.
  • Oversee and supervise employees assigned to the Program Manager’s program. This includes, but is not limited to training, scheduling, counseling and discipline, coaching and mentorship, and performance evaluations.
  • Provide input and/or make decisions regarding the employment status of direct reports, including but not limited to promotion, demotion, disciplinary action, suspension, removal from the program, or termination.
  • Communicate with guardians, social workers, vocational providers, and medical personnel of members on a routine basis via phone, email, and/or in-person meetings. Keep care team members informed of all pertinent information relevant to the member’s care. Submit incident reports and medication error reports to the care team in a timely manner and in compliance with DHS regulations.
  • Maintain the cleanliness and general upkeep of the program house, yard, and/or vehicle. Assign cleaning and upkeep tasks to employees, and periodically follow up to ensure tasks are being completed to ILC standards. Notify the CEO of any maintenance or repair needs at the home. Monitor the condition of the company vehicle. Schedule vehicle maintenance and repair needs in a timely manner.
  • Acquire necessary supplies, equipment, or services for the program and its members. Coordinate with members’ care teams to obtain necessary and/or physician-prescribed medical supplies, equipment, and services. Monitor the program for grocery and household supply needs. Ensure a sufficient supply of household goods (i.e. toilet paper) and standard food staples (i.e. fruits and vegetables, bread, milk) are available in the home for employee and member use. Complete regular grocery shopping and/or assign a suitable employee to do so.
  • Perform additional duties as indicated by program or member needs, or as directed by executive management

Supervisory Responsibility:

This position is directly responsible for the assigned ILC, LLC program and the employees who work at that program.

Work Environment:

This position operates in an office environment, in a home environment and/or in the community. This role routinely uses standard office equipment such as computers, printers, fax machines, copiers, filing cabinets, and phones. This role also regularly operates common medical equipment, such as Hoyer lifts and electric or manual wheelchairs. This role regularly uses a car or wheelchair-accessible van to transport members.

Physical Requirements:

The physical demands described here are representative of those which must be met by an employee to successfully perform the essential functions of this job, with or without reasonable accommodations.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee may need to hear individuals whose speech is difficult to understand. Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception, and ability to adjust focus. This position requires the ability to regularly lift objects, up to about 20 pounds; it may occasionally require the ability to lift more than 20 pounds. The employee is frequently required to sit; stand; walk; use hands and fingers to handle, feel, or operate objects, tools, or controls; and reach with hands and arms. Additional physical requirements of this position include:

  • Balancing: maintaining balance to prevent falling while performing body movements, including when working with uncontrolled, intermittent movements, or another person

  • Carrying/lifting: raising, lowering, or transporting an object or person from one level to another using hands, arms, and shoulders. Except in event of emergency the assistance of another staff member is used when moving a person

  • Crouching: bending the body downward and forward by bending the legs and spine
  • Reaching: extending the hands and arms in any direction, including above the shoulder level
  • Pushing/pulling: exerting force to move an object away or towards the body

Position Type/Expected Hours of Work:

This is a full time position. Regular days of work are Monday through Friday; hours of work are flexible. Occasional evening and/or weekend work may be required as job duties demand.

Travel:

Travel is primarily local during the work day. Some out-of-area or overnight travel may be expected on an occasional basis, such as for training and development purposes.

Education and Experience:

The minimum requirements for this position are as follows:

  • Associate’s Degree and two years related experience, or an equivalent combination of education and experience, are required
  • A valid driver license and clean driving record

The following qualifications are preferred, but not required:

  • Experience providing community supports to individuals with disabilities, or in a related human service field
