Job Description:

Nystrom & Associates, Ltd. is a team of mental health professionals that specialize in psychiatric evaluations, clinical social work, and family therapy. We are committed to helping patients in our community who are experiencing personal, emotional, marriage, family, or psychological problems. Founded in 1991, we offer an intensive outpatient chemical dependency program, psychological testing, and behavioral health and medication management services.

Leadership and Team Management:

Promotes patient's health by assessing, developing, planning, implementing, and evaluating day-to-day and long-term management of the Psychiatry Triage Team.
Supervise a team of triage staff, providing guidance, support, and fostering a positive work environment.
Conduct regular performance evaluations, identify training needs, and provide ongoing coaching and development opportunities.
Promote teamwork, collaboration, and effective communication within the triage team and with other departments.
Participate with Human Resources and other appropriate team members to support onboarding and offboarding activities for Triage Team members.

Triage Process Oversight:

Develop and implement triage benchmarks, protocols, and guidelines to ensure consistent and efficient patient assessments, clear and concise documentation, and all other day-to-day functions.
Review and update triage policies and procedures in accordance with industry best practices and regulatory standards.
Monitor the triage process to ensure accurate and timely prioritization of patient care based on severity and acuity levels.
Collaborate with providers and other team members to ensure seamless coordination of patient care and optimize resource utilization.
Maintains Triage related policies and procedures, developing workflows, training tools and resources.

Clinical Decision-Making:

Apply advanced clinical knowledge and critical thinking skills to assess and evaluate patient symptoms, complaints, and vital signs.
Make informed and accurate decisions regarding appropriate care pathways, including recommending emergency services, urgent care, or scheduled appointments.
Provide consultation and guidance to triage team members for complex or ambiguous cases, ensuring optimal patient outcomes.

Documentation and Reporting:

Ensure accurate and thorough documentation of triage assessments, recommendations, and outcomes in electronic medical records.
Compile and analyze data related to phone call volumes, triage outcomes, strategic plans, and key performance indicators to identify trends and areas for improvement.
Prepare regular reports on triage activities, including productivity, quality metrics, and patient satisfaction.
Oversight and ownership of team day-to-day functions.

Requirements

Bachelor's degree in nursing (BSN) from an accredited institution.
Current and unrestricted Registered Nurse (RN) License.
Proven experience in nursing triage or behavioral healthcare settings.
Minimum of 3 years prior leadership or supervisory experience.
Excellent clinical assessment and decision-making skills.
Strong knowledge of medical terminology, disease processes, and treatment options.
Proficiency in using electronic medical record (EMR) systems and other relevant healthcare technologies.
Exceptional organizational, interpersonal, and communication skills.
Ability to multitask, prioritize, and work effectively under pressure.
Demonstrated commitment to patient safety, quality care, and continuous improvement.
