SCOPE OF ROLE

Reporting to the Director, Clinical Quality, the Manager, Clinical Record Integrity will continuously improve the quality of care provided to our patients. The Manager, Clinical Record Integrity will review, analyze and evaluate patient records for completeness and compliance with applicable company guidelines, statutory requirements, established standards of care, applicable payer rules, and HIPAA requirements. The Manager, Clinical Record Integrity will work collaboratively with all departments and communicate opportunities for improvement.

HOW YOU'LL SUCCEED:


  • Collaborate with the Education and Training Department to develop training materials and conduct staff training on clinical records; prepare and report review outcomes and work with supervisory staff to ensure compliance and improvement.

  • Offer insights, observations and suggestions for quality improvement to relevant teams. Offer quality improvement suggestions on guidelines, policies and procedures; facilitate quality improvement plan development and implementation.

  • Develop, implement, and manage a Counselor Peer Review process.

  • Create, design, and carry out targeted audits of patient records.

  • Contact clinicians to request information or changes where warranted, bringing records into compliance.

  • Work collaboratively with the Legal and Compliance department to assist with investigative audits. Collaborate with Human Resources to assist with performance related audits and/or investigations.

  • Conduct auditing and reporting to ensure organizational compliance with appropriate policies concerning confidentiality and conflict of interest, as well as all HIPAA, HRSA, CARF, and OMHAS requirements concerning patient /staff confidentiality and privacy issues.

  • Encourage engagement and participate in the education and orientation of Signature Health staff regarding medical record documentation, accuracy and integrity .Provide follow-up, training, and /or other support to assure medical record quality.

  • Review documentation for coding, diagnosis and compliance with applicable procedures.

  • Maintain electronic record compliance spreadsheets and audit tracking.

  • Participate in continuing education activities related to medical record quality.

  • Partner with the Director of Clinical Quality to streamline review processes and improve review tools.

  • Other duties as assigned.

Requirements

KNOWLEDGE & EXPERIENCE:


  • Bachelor's Degree required.

  • 6 years of experience in a clinical setting with demonstrated proficiency of medical record and electronic health record system analysis required.

  • Working knowledge of HIPAA Privacy and security rules required.

  • Demonstrated communication skills.

  • Demonstrated proficiency in basic computers skills, Windows, Outlook, Word, PowerPoint, Internet Explorer, etc.


WORKING CONDITIONS:

  • Work is normally performed in a typical interior/office/clinical work environment.

  • While hours of operation are generally standard, flexibility to work evenings and extended hours may be required.

  • Requires periods of sitting, standing, telephone, and computer work.

  • Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology.

  • Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology.

  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.

  • Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted.

  • Possible exposure to blood borne pathogens while performing job duties.

  • Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday.

  • Sufficient dexterity to operate a PC and other office equipment.
