We are currently hiring for
the following roles:
Supervisor, Care Manager, Recovery Manager &
Assessor to work primarily
in Erie, Ottawa, and
Sandusky counties.
Responsibilities differ depending on the position.

Duties may include
assessments, outreach,
verification and/or care management services for
individuals requiring community-based long-term care or other Home & community-based services.

Qualifications:
Registered Nurse, Licensed Social Worker (LSW, LISW), or a bachelor's degree in Counseling, Psychology, or a related field, with a minimum of three (3) prior experience in home & community-based services or case
management. Reliable
transportation, insurance,
and a valid driver's license. Ability to pass an FBI/BCI check and a Motor Vehicle Record (MVR) check annually
