Job Description:

Assisting patients and families navigate during their most challenging moments is a job not everyone can do. Medical Social Workers (CSWA/ LMSW) serve an exceptionally critical function during those moments that are essential to our team. Be empowered by joining a team of hospice clinicians making a difference to patients.

We are looking for a compassionate and motivated licensed social worker who will provide a tender supportive care experience to the patients and family during their visits. As a locally-owned hospice agency, we aim to offer a hospice service done the right way with our tender hearts. Come and join our team!

Job Summary
Provides onsite psychosocial assessment and plan of care - includes emotional, social, financial, and environmental resources and to identify appropriate psychosocial problems, interventions, and goals for the patient’s plan of care. Offers support to patients and guides them toward comfortable end-of-life care.

Qualifications

  • Must have a license (CSWA, LMSW) or in the process of getting licensed and a master of Social Work degree from a school of social work. New graduates are welcome in our team!
  • Must have an excellent problem-solving, critical thinking and social skills and the ability to build trusting relationships not only with the patient but with all the care team members involved in the care
  • Should be skillful in organization and in the principles of time management and have knowledge of basic nursing skills
  • The ability to communicate well, both verbally and in writing.
  • Must have the ability to demonstrate self-confidence and a positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement.
  • Must be able to fulfill all training requirements per Hospice’s policy.
  • Must be able to perform roles within the safety standards per the Hospice’s Policy and Procedure and licensing standards.
  • Must be able to represent Tenderly Hospice’s Tenderhearted Principles, Mission and Vision.
