Job Title: Home Health Registered Nurse
Duties & Responsibilities: Home Health Registered Nurse sought for 13-26 week assignment, with the potential to extend. Schedule would be 5x8 days, Monday-Friday, with on-call once weekly as needed and an every 3rd weekend rotation. There is a minimum productivity expectation of 30 points per week. Patient caseload is 25-30 patients for case managers and 6-7 visits daily for non-case managers. Mileage is reimbursed at $0.50/mile.
Minimum Requirements:
- OH or Compact RN license
- 1+ year home health experience within the past 3 years
- BLS
- HCHB experience preferred
- OASIS experience strongly preferred
