Redwood Cove Healthcare Center is seeking a dedicated and detail-oriented LVN MDS Coordinator to join our team. • Oversee the MDS (Minimum Data Set) process, including assessment, data entry, and submission, to ensure compliance with federal and state regulations.


• Collaborate with the interdisciplinary team to develop individualized care plans for residents based on their assessments and needs.


• Monitor and review the accuracy of resident assessments and care plans, making adjustments as necessary.


• Participate in care plan meetings, providing input and recommendations based on resident assessments.


• Ensure proper documentation and record-keeping for all MDS-related activities.


• Train and educate staff on MDS processes and regulations as needed.

• Current California LVN license.


• Previous experience as an MDS Coordinator or in a similar role in a skilled nursing facility.


• Strong knowledge of MDS 3.0, RAI process, and CMS guidelines.


• Excellent communication, organizational, and analytical skills.


• Ability to work effectively both independently and as part of a team.
