Job Description:

JOB RESPONSIBILITIES:

Handles incoming and outgoing medical record requests for patients, providers, insurance companies, and/or law firms. Follows HIPAA guidelines to ensure all requests are done properly and follows all JMH standards when completing a request.

Searches for requested records using the EMR system. Assembles all requested records and ensures they are organized by timeline. Uses proper documentation for all outgoing records to show the request has been completed. Scans all incoming records into the patient's chart and sends these records to the appropriate provider. Faxes and/or mails medical records when requested based upon the size of the document.

Manages incoming referrals. Schedules incoming referrals based upon type of referral, urgency, patient preference, and availability on the providers' templates. Works closely with the· nurse to ensure any urgent incoming referrals are worked into the providers' schedules and communicated with the provider. Documents when patients-are scheduled in the EMR system. Communicates with the referring office with updates to the patient's appointment.

Verifies insurance and insurance eligibility for all incoming referrals. Runs eligibility using the EMR system and/or the payer's website. Ensures a suitable referral has been sent based upon the patient's type of insurance. Tracks the number of visits allowed for all Tricare referrals. Communicates with the referring office when more visits are needed for a Tricare referral, along with obtaining a new referral if a new diagnosis or surgery arises.

Works closely with providers to obtain their approval on all outgoing medical records. Works with other physicians' offices, law firms, and/or insurance companies to ensure their requests for records are accurate and complete. Follows up with outside facilities to ensure records were received or sent. Ensures incoming records are provided to the appropriate provider.

Tracks all transfers in and out of the office and ensures all records have been sent to the appropriate office. Works with the billing department to ensure they note the patient as a transfer and bill the patient accordingly.

Ensures all medical records necessary are present and attached to all incoming referrals in the EMR system. Sends all records obtained to the appropriate provider prior to the patient's appointment. Communicates with the referring office when more records are needed prior to the appointment.

Sorts incoming faxes to find any medical records, requests, and/or incoming referrals. Attaches documents received to the patient's chart and sends them to the appropriate provider when required.

Handles all incoming and outgoing phone calls from patients or other facilities pertaining to medical records, medical record requests, and incoming referrals.

Creates cover letters for medical records sent to law firms or insurance companies. Creates a charge based on the medical records fee schedule when necessary. Creates invoices and tracks payments for medical records requests when required.

Assists with front office duties, such as, answering phones, check-in, check-out, faxes, scanning, registering and scheduling of patients.

Clearly communicates and continuously supports the Mission of Values of JMH.

Conducts all activities in compliance with applicable laws, regulations, standards, and JMH policies and procedures, including, Blood and Body Substance Precautions.

Performs other duties as assigned by supervisor.

EDUCATION, EXPERIENCE AND TRAINING:

High school diploma or equivalent required. Minimum of 2 years of experience in a physician practice or medical setting required. Medical record experience preferred.

Monday- Friday; 8am-5pm.
80 hours per two-week pay period.
