Job Description:

Facility: Wellstar Primary Care - Acworth (3501 Baker)



The Lymphedema Physical Therapist evaluates, plans, organizes, and implements rehabilitative programs to enhance mobility, alleviate pain, build strength, and prevent or reduce deformities resulting from disease or injury. Services are delivered across the full care continuum with a focus on cost efficiency, quality, and customer satisfaction, while ensuring compliance with all relevant regulatory standards.



Core Responsibilities and Essential Functions:




Patient Assessment:



  • Collaborate with physicians to develop and evaluate treatment programs, care plans, goals, and discharge plans.


  • Create a care plan with input from the patient and/or family.


  • Develop a realistic discharge plan that incorporates patient and/or family goals.





Treatment Planning and Provision of Care:



  • Implement the physical therapy treatment plan.


  • Select appropriate treatment activities to help patients achieve their goals.


  • Adjust goals and care plans with input from patients and their families.


  • Provide ongoing education and training to patients and families.


  • Offer discharge instructions and referrals to community resources as needed.


  • Serve as a resource for staff, families, and physicians regarding rehabilitation services and equipment.





Documentation:



  • Document evaluations and care plans according to departmental guidelines.


  • Record each treatment session following departmental protocols.


  • Complete and submit all required documentation, including family education and discharge paperwork, in accordance with facility and program guidelines.

Required Minimum Education:



  • Bachelor's, Master's, or Doctorate in Physical Therapy





Required Minimum Licenses and Certifications:



  • Physical Therapist license


  • Basic Life Support (BLS) certification





Required Minimum Skills:



  • Effective communication


  • Customer service focus


  • Team-oriented


  • Basic computer skills
