Social Worker – Outpatient Psychiatric Case Management (FT - Days) at Excelsior Springs City Hospital in Excelsior Springs, Missouri

Job Description:

Perform the initial intake for therapy/counseling.Be able assist patient and collaborate with providers with resources regarding social determinants of health such as food, housing, transportation.Be able to assist patients and collaborate with providers with resources regarding Medicaid, Medicare, insurance, legal services, medical needs.Work with providers and therapists to ensure patients have a current DPOA, guardianship, and the current parent plan for patient being seen in the clinic.Have a current bonding for being a notary.Provides brief problem-solving therapy or counseling services to patients.Be a support to family members dealing with mental illness in the family.Conducts interviews to determine the appropriate services to offer mental health patients and their families.Develops care plans that include counseling, support services, treatment methods and referrals.Responsible for timely and accurate completion of medical records, including assessments, progress notes outcome measures and billing entries.Maintains patient records, prepares reports, and monitors progress toward treatment goals.Performs other duties and responsibilities as assigned.
