The Registered Nurse provides direct nursing care in accordance with established policies, procedures and protocols of the organization. The nurse involves protecting, promoting, and optimizing health and abilities through a patient-centered approach. Responsibilities include preventing illness and injury, alleviating suffering via accurate diagnosis and effective treatment, and advocating for patients and their families. The role demands a commitment to ensuring all actions and decisions are aligned with the principles of patient-centered care.
Essential Job Functions:
Performs bed side palliative care of patient/family.
Collects and analyzes data to improve palliative care services
Anticipates and assists with symptom management, including pain control.
Participates in family support, teaching, discharge planning and continuity of care across sites of care, in collaboration with primary care team.
Coordinates and facilitates Interdisciplinary Team meetings and Patient/Family Care Conferences.
Participates in formal and informal education for all disciplines.
Assists in all facets of care coordination for palliative care referrals.
Establishes and maintains communication and utilizes educational opportunities with patients and other significant individuals.
Performs other duties as assigned or needed to meet the needs of the department/organization.
