ATA Services, Inc is currently seeking to hire a Family Connects New Mexico Registered Nurse to work on a temporary assignment with the State of New Mexico Department of Health - Family Connects New Mexico (FCNM) program.

  • Start Date: ASAP
  • Pay Rate: $43-$69 per hour depending on experience, with weekly pay
  • Schedule: 40 hours per week
  • Workspace: Office, Home visits and the community.
  • Availability to work weekends as required.

Family Connects New Mexico Registered Nurse

Introduction

The Family Connects New Mexico (FCNM) program is designed to provide universal home visiting services to families with newborns in a specified catchment area. The program aims to support families during the critical early stages of a child's life by assessing needs across 12 domains of physical and psychosocial well-being and connecting families with community resources to address these needs. The Registered Nurse (RN) in this role will be instrumental in delivering these services, working within a multidisciplinary team to ensure comprehensive care for families.

JOB DUTIES:

Home Visiting and Family Support

  • Provide home visiting services to all families with newborns in the designated catchment area, following the evidence-based Family Connects NM home-visiting protocol.
  • Conduct health and physical assessments of both the mother and baby during home visits to determine their well-being.
  • Assess family needs across 12 domains of physical and psychosocial well-being, including areas such as depression, substance abuse, and domestic violence.
  • Establish a trusting relationship with families, working collaboratively to develop a family support plan with specific goals and activities that address the family's strengths and needs.
  • Educate caregiver(s)/parent(s) on the universal needs of families with newborns, emphasizing that every family has different levels of need and working to de-stigmatize home visiting.

Scheduling and Coordination

  • Follow the Family Connects scheduling protocol to arrange home visits for new mothers, including visits to target area hospitals as needed.
  • Work within a team of clinical and non-clinical staff to schedule, visit, and support families in the community.
  • Maintain eight open visit slots and conduct and document at least six home visits per week, ensuring flexibility to respond to client needs, including occasional evening or weekend visits.

Documentation and Record Keeping

  • Keep accurate and up-to-date records of client observations and activities as required by the Family Connects protocol.
  • Document information for family assessments and program evaluation, ensuring that all records are entered into the computerized system in real-time during home visits.
  • Complete documentation in compliance with state guidelines, ideally within 24 hours of the encounter.

Referral and Community Connection

  • Work with the FCNM team to ensure appropriate referrals and connections to community resources based on the assessed needs of families.
  • Assist families in making and attending appointments with doctors, social service agencies, and other community organizations, acting as a liaison and family advocate as required.
  • Apply working knowledge of parent-child interaction, child-maternal health, child development, and child abuse/neglect to help families improve parenting skills, increase supports, reduce stress, and address the needs of both the child and parents.

Team Collaboration and Professional Development

  • Actively participate in weekly team meetings, clinical supervision, staff meetings, and other meetings as required.
  • Support the professional development of other staff members and contribute to the growth of the Family Connects program.
  • Work with the nurse supervisor to implement practice improvement and clinical development plans.
  • Demonstrate proficiency in time management, organization, and multitasking within the scope of work.

Service Delivery Environment

  • Provide services primarily in the home but also in clinics, auxiliary, and community sites as needed.
  • Work in an office setting with exposure to Visual/Video Display Terminal (VDT), extensive personal computer and phone usage, and periods of office noise, bending, standing, and lifting.
  • Must be capable of lifting up to 50 lbs.
  • Deliver services in a culturally sensitive manner, accommodating the needs of diverse racial and ethnic populations, including those who do not speak English.

Qualifications

  • MUST possess and maintain a current New Mexico Registered Nursing license or be eligible for licensure in accordance with the Nursing Practice Act.
  • Preferred skills include Spanish language proficiency, clinical experience related to domestic violence, substance abuse, and/or depression, and at least one year of experience in child/maternal health, social work, or a related field.
  • Certification as a Lactation Educator or Consultant is preferred.
  • Ability to work in a team with both clinical and non-clinical staff.
  • Proficient in using laptops and other technology for in-home assessments and interventions.
  • Must maintain current CPR certification.
  • Availability to work weekends as required.
