Position: Primary Care Nurse Practitioner Schedule: Full-Time, Monday – Friday, 8:00 AM – 4:30 PM (No Weekends) Location: San Diego, California Job Type: Full-Time
Description: Join our dedicated team at St. Paul’s PACE, a respected San Diego-based non-profit organization committed to providing high-quality, compassionate primary care in an outpatient setting. As a primary care provider, you will play a crucial role in managing both acute and chronic conditions for our elderly participants, collaborating with an interdisciplinary team, and enhancing the overall well-being of our community.
Key Responsibilities:
Deliver comprehensive primary care, managing both acute and chronic conditions.
Refer participants to in-house providers, including behavioral health services.
Review and manage imaging and lab results to ensure accurate diagnosis and treatment.
Prescribe medications and develop treatment plans for various conditions.
Accurately document all patient interactions in the electronic medical record.
Conduct Skilled Nursing Facility (SNF) as necessary.
Actively participate in PACE Interdisciplinary Team (IDT) meetings.
Educate participants and caregivers on health management and preventive care strategies.
Adhere to PACE regulations and organizational policies, including maintaining up-to-date credentials and certifications.
Qualifications and Requirements:
Education: Graduate of a professional Nurse Practitioner or Physician Assistant program. Must be licensed in California and board-certified.
Experience: New graduates are encouraged to apply. While a minimum of 1 year of experience as a Nurse Practitioner or Physician Assistant following clinical training is preferred, we are open to considering candidates with less experience, particularly those who have completed training in a community health center or similar setting.
Skills:
Extensive knowledge of primary care for frail older adults.
Ability to work effectively with diverse social, cultural, and economic groups.
Proficiency in electronic medical record documentation and basic computer literacy.
Strong communication and organizational skills.
Leadership experience is an advantage.
Bilingual capabilities (English/Spanish) are highly desirable.
