Salary: $140,000.00 USD Annually - $150,000.00 USD Annually

Job Title: Nurse Practitioner (NP) - In-Home Wellness Risk Adjustment
Location: Clark County, WA
Shift: Monday-Friday, 8:00 AM - 5:00 PM (typical corporate schedule; flexibility available)
Salary: Highly competitive and based on experience.
Experience Requirement:
- Accepts new grads but requires relevant experience as an RN with either homecare or patient care.
- Previous in-home risk assessment experience is a plus but not required.
Responsibilities:
- Conduct annual wellness visits and health assessments for a population with chronic conditions.
- Deliver patient health education opportunities.
- Assist in closing quality care gaps (e.g., screenings and labs).
- Collaborate enthusiastically to enhance care delivery.
Qualifications:
- Valid unencumbered NP License for the state where you will be working.
- Any certification/specialty accepted.
- Patient care experience.
- Bilingual skills are a plus.
- W2 full-time employment.
Travel Expectations:
- Most of the work within the county, but occasional travel to other areas of the state for assessments (assignments given 2 weeks in advance).
- All travel expenses (including hotel and food allowance) are covered.
