Job Specifics:



The Clinical Specialist will assist the Rehab Team Leader with specialty program development & external marketing; participate in multidisciplinary rounds, provide formal & informal in-services /educational offerings for staff and the community; assess & track outcomes and develop plans, based on clinical outcomes & metrics and mentor peers. In addition, this position demonstrates competency in the performance of duties and responsibilities related to patient evaluation; development of care plans; administration of therapeutic interventions and documentation of clinical care.


1. Partnership and collaboration within specialty program.
a. Provide formal and informal in-services or training sessions for appropriate staff.
b. Participate in specialty clinical rounds as appropriate.
c. Assist the Team Leader and/or Manager in the marketing of the specialty program outside of the department to physicians, providers, WDH clinic staff, and the community.
d. Participate in a minimum of one community lecture, family or patient educational program/support group.
e. Present a minimum of one educational in-service to clinical staff on agreed upon topic with the Program Specialist.
f. Assist Team Leader and/or Manager in assessing and tracking outcomes.
g. Assist Team Leader and/or Manager in Performance Improvement plans to address opportunities for improvement within the department based upon clinical outcomes and metrics.
h. Participate in a minimum of one student teaching opportunity.
i. Assist Team Leader and/or Manager in developing/updating protocols that are supported by evidence based best practices.
j. Serve as a mentor to peers, and provide co-treatment/consultation on plans of care for peer's patients as needed.
k. Provide Team Leader and/or Manager input on updating the inpatient and outpatient specialty clinical pathways.
l. Maintains requirements that are necessary to keep specialization current.
m. Attends a minimum of 6 CEU's in area of specialty per year.
n. Majority of patient caseload is working directly with patients within given specialty.


2. Demonstrates competency in the performance of duties and responsibilities related to patient evaluation and development of care plan.
. Selects tools, tests and measures appropriate to patient age and diagnosis.
a. Recognizes deviations from anticipated assessment findings.
b. Establishes and prioritizes functional, measurable goals and includes time frames.
c. Establishes treatment plans based on examination findings.


3. Demonstrates competency in the performance of duties and responsibilities related to the administration of therapeutic interventions.
. Provides care that maintains patient confidentiality, comfort and safety.
a. Administers therapeutic agents and exercise programs according to clinical procedures.
b. Selects, provides and instructs in the use of adaptive equipment.
c. Assesses patient response to intervention and adjusts care appropriately
d. Performs reassessment and goal adjustment according to policy.
e. Instructs patient/family in the patient's therapeutic program, including discharge planning.


4. Demonstrates competency in the performance of duties and responsibilities related to the documentation of clinical care.
. Maintains medical record including assessment, interventions and clinical response.
a. Obtains and reviews all required insurance authorizations and physician signatures.
b. Documents patient outcomes that reflect goal achievement.
c. Completes physician progress reports and discharge summaries according to department policy.
d. Records accurate patient charges on a daily basis.


5. Demonstrates competency in the performance of duties and responsibilities related to the communication of clinical and operational information.
. Participates in daily inpatient discharge planning rounds.
a. Provides patient status report for weekend/vacation coverage and rounds.
b. Communicates with other providers regarding the needs and capabilities of each patient.
c. Communicates patient status report according to insurance company requirements.
d. Provides verbal and written information to patient/family.
e. Establishes consistency with physician expectations for departmental program goals.


6. Demonstrates competency in the performance of duties and responsibilities related to receiving and providing education.
. Identifies educational needs and seeks opportunities for improvement.
a. Attends a minimum of one out of hospital rehabilitation program.
b. Facilitates rehabilitation staff education regarding out of hospital education program.
c. Maintains continuing education record semiannually.
d. Completes clinical competencies annually.
e. Serves as a resource for other departments and staff.
f. Provides regular formal & informal in-service training for staff.


7. Demonstrates competency in the performance of duties and responsibilities related to the supervision of support staff.
. Assigns patients to Assistants and communicates all pertinent patient information
a. Re-evaluates Assistants patients according to department policy.
b. Provides continuous consultation with Assistants.
c. Directs and supervises aides.
d. Directs and supervises secretarial staff relating to daily operations.



Qualifications

Experience Minimum Required

• Min of 2000 hrs. of clinical practice in the specialty area, 25% which has occurred in the last 3 yrs.



Education Minimum Required

• BA or BS in Physical Therapy, Occupational Therapy, or Speech Therapy



Training Minimum Required

• Post professional certification/specialization in area of specialty supported by National Board Exam or successful completion of Fellowship/Residency in area of specialization.



Special Skills Minimum Required

• Advanced clinical reasoning and mentorship exp



Licensure and/or Certifications Required

• NH License Physical Therapist
• AHA or ARC BLS within 2 weeks of hire
