Job Role Summary
Coordinates, facilitates, implements and participates in core services management and quality improvement initiatives. Develops process improvement plans with IHS Management and Evaluation Team to analyze, monitor and ensure high levels of quality services and performance. Assists with the achievement of Indianapolis Healthy Start core services (case management, outreach, and health education) cost effective, high quality services. Identifies opportunities for improvement, makes recommendations for change, and leads the implementation of best practices to meet established goals. Supervises the IHS Preconception, Prenatal and Interconception (PPI) Lead. Provides coaching and direction in the management of IHS PPI staff. Develops a process of continuous quality improvement to ensure continuous updates and improvements of the policies and procedures as related to the electronic data management system are current and efficient.
Associated Job Duties
Provides supervision to the IHS Lead Case Manager, Subcontract Case Manager
• Performs IHS Prenatal/Perinatal Lead duties, as needed
• Serves as an educator, communicator and resource to sub-contractors, partners, collaborators and participants
• Responsible to assist with holding subcontractor accountable for Scope of Work deliverables
• Manage the department electronic data management system (includes designing, revising, monitoring and training on procedures that monitor and evaluate the quality and fidelity of the program)
• Leads Evaluation Team meetings
• Assist the management team in developing program standards of care and quality indicators utilizing evidence based practices
• Ensure appropriate data is collected and entered into the electronic EHB and/or program data system to track progress on the quality indicators.
• Provides monthly and quarterly audits to IHS staff
• Monitor and report program quality and fidelity of program services and outcomes at theparticipant, site and overall program levels utilizing the data entered into the system
• Monitor and evaluate the success of core services; modify service(s), as needed, to maintain ongoing benefit to both the needs of the catchment area residents and community.
• Liaison with internal/external partners on program and operational matters
• Develop a process of continuous quality improvement to ensure continuous updates and improvements of the policies and procedures as related to the electronic data management system.
• Utilizing statistical software, develop and maintain statistics for program outcome, evaluation, research and grant purposes on a regular basis as requested by program administration. Work closely with HHC epidemiology department staff, external evaluators and consultants for data analysis.
• Conduct review of literatures to identify evidence based practices in this area. Assist in writing publishable peer-reviewed articles related to program quality and outcomes.
• Assist in creating and writing the sections of grant applications, inclusive of case management and evaluation
• Attend, prepare and present at various local and national related workshops and conferences
• Attends Healthy Start and Indianapolis Healthy Babies Consortium Meetings
• Submit a minimum of (2) Plan-Do-Study-Acts (PDSA)s per grant year
• Works closely with IHS Administrator to ensure program objectives are being met
• Demonstrate Friendly Access by supporting corporate values: Community, Leadership, Excellence, Accountability and Respect (CLEAR)
• Trained at the appropriate(NIMS) and (ICS) level
• Maintains confidentiality of all client records and information as specified by theHealth Insurance Portability and Accountability Act of 1996
• Participates in a minimum of two H.O.P.E. events totaling a minimum of eight (8) hours annually
• Participate in public health emergency preparedness exercises and response to public health emergencies, as directed
• Performs other duties, as assigned
