Job Description:

JOB SUMMARY: As the lead of our SAMSHA grant funded GBHI program, this role will oversee the day-to-day operations of the GBHI program services, supervise program staff, and monitor program deliverables. The program provides helps communities expand and strengthen treatment and recovery support services for individuals (including youth and families) experiencing homelessness who have substance use disorders or co-occurring mental and substance use disorders. Our program also assists with Housing resources.

DUTIES AND RESPONSIBILITIES

Supervision and Leadership
  • Supervise housing specialists, and substance use counselor in the full range of services.
  • Provide weekly individual supervision and biweekly group supervision to staff and assigned interns.
  • In collaboration with the Director, support all housing staff through clinical supervision both individually and in a group setting.
  • Provide direct services to tenants in the absence of staff due to vacancies, leaves, vacation, etc.
  • Responsible for the overall implementation and management of the Grants for the Benefit of the Homeless Individuals (GBHI) program, including outreach to build census and oversight of program staff and deliverables.
  • Supervise the identification and prioritization of the homeless population for behavioral health services.
  • Oversee and collect baseline and ongoing program data (outcome assessments).

Service Provision & Referrals
  • Supervise concrete and clinical referrals, including outreach and street outreach.
  • Counsel patients using Motivational Interviewing, Harm Reduction, Trauma-Informed Care, and Contingency Management.

Supervise the assessment of patients' medical, behavioral, and substance use needs, functional abilities, concrete needs (e.g., housing, finances, legal), social and family support, language and immigration issues, and barriers to care.

Case Management and Assessments
  • Ensure that all client assessments and service plans are thorough, client-centered, and completed on time.
  • Conduct intakes, assessments, and treatment plans. Assign cases and tasks as needed.
  • Supervise program staff and complete all required monthly and other reports.
  • Ensure all services are provided in a culturally and linguistically appropriate manner.
  • Complete annual and ongoing (as needed) mental health evaluations for YAF tenants.
  • Expert with chemical dependency evaluations, as well as chemical abuse/dependency aspects of treatment plans and treatment plan updates

Program Coordination and Outreach
  • Participate in department and program meetings, including Weekly Team Meetings, Monthly Tenant Meetings, etc.
  • Effectively use tools such as safety plans, community meetings, psycho-education groups, team meetings, red flag reviews, treatment planning conferences, and self-care to create a nonviolent and trauma-sensitive culture.
  • Conduct Outreach to shelters, probation offices, prisons/jails, etc., to build census.
  • Allocate up to 50% of your role to participating in outreach efforts.

Other Responsibilities
  • Other duties as assigned based on program needs.
  • Duties may be outside regular work hours and may include evenings and weekends when needed.

KNOWLEDGE, SKILLS, & ABILITIES:
  • Strong working knowledge of casework practice with exceptional verbal and written communication skills.
  • Flexibility to work varying hours, including some evenings, with on-call availability as needed, and the ability to provide emergency coverage when required.
  • Commitment to a strength-based, youth development, and trauma-informed approach.

EDUCATION:
  • MINIMUM: Master's degree in social work, Mental Health Counseling, or other related field required.
  • Current NYS LMSW/LCSW/LMHC/LCAT required

REQUIRED SKILLS:
  • MINIMUM: 2 to 4 years of supervisory experience required.
  • MINIMUM: 1 year of providing services to individuals experiencing homelessness and substance abuse.
  • Expertise in working with individuals living with psychiatric illnesses
  • Experience in community-based work and a strong understanding of community resources.
  • Excellent written and verbal communication skills.

PREFERED SKILLS:
  • Proficiency in Microsoft Office applications, including Outlook, Word, Excel, and PowerPoint.
  • Bilingual candidates are strongly encouraged to apply. (Spanish and Haitian-Creole preferred)
