What You'll Do:
Responsibilities
Completing accurate MDS assessments and comprehensive care plans.
Collaborates with the Interdisciplinary Team to assess the needs of the residents.
Monitors MDS and care planning documentation for all residents; ensures documentation is in the medical record to support MDS coding.
Informing DON of persistent issues related to non-compliant documentation.
Always Protecting the confidentiality of Resident & Facility information.
Involvement in UR, CMI and Triple check meetings.
What You Bring
Qualifications
Must have prior experience with MDS.
Must be proficient with Massachusetts Case Mix and PDPM.
Prior experience with PCC, Sigma and Net Health preferred.
Long-term care experience is required.
Must be highly organized, professional & motivated.
Should have solid computer skills.
Must have an active Massachusetts license.
Regional experience preferred
RAC-CT preferred.
License/Certification:
RN (Required)
