Job Title: Palliative Care RN Case Manager



Duties & Responsibilities: Pallative Care RN Case Manager sought for 13 week assignment, with the potential to extend. Schedule would be 5x8 days, 830a-5p, with an every other weekend rotation. The coverage area is South and North San Diego County with an average of 50-60 miles/day.



Minimum Requirements:

• CA RN license


• BLS


• 1+ year experience in-field Home Health


• Palliative care experience


• OASIS experience


• Valid Driver's license and Auto Insurance
