Job Summary The Food as Health Alliance has an immediate opening for a Referral Coordinator. The coordinator will gather information concerning patient’s clinical and referral needs and assist patient(s) with completing enrollment paperwork for food as medicine program over the phone daily and will assist patients in problem solving potential issues related to the food as medicine program, financial or social barriers (e.g., request interpreters as appropriate, document transactions, transportation services or IT assistance.
Skills / Knowledge / Abilities Organizational skills, proficient in MS Office skills, familiar with RedCap or other data management system, Excellent communication skills – both verbal and written. Strong problem-solving skills and detail oriented. . Ability to follow instructions, comply to HIPPA laws, and work as a productive member of a collaborative team. Successful candidates will demonstrate a superior level of customer service to all constituents during each interaction.
