• Complete and coordinate the Minimum Data Set (MDS) assessments for residents in compliance with federal and state guidelines.


• Work closely with nursing, therapy, and other interdisciplinary teams to ensure accurate and timely assessments.


• Ensure accurate documentation to support reimbursement and compliance with regulatory standards.


• Conduct care planning meetings and collaborate with staff to ensure individualized care plans.


• Maintain up-to-date knowledge of MDS regulations and Medicare/Medicaid reimbursement guidelines.

• Valid LVN license in the state of California.


• Previous experience in MDS coordination in a skilled nursing or long-term care setting.


• Strong attention to detail, organizational, and communication skills.


• Knowledge of MDS 3.0 and understanding of Medicare/Medicaid regulations.


• Ability to work effectively under pressure and meet deadlines.

• Competitive salary, range btwn $44 and $48/hr
