Job Description:

WELLNESS COORDINATOR

 

POSITION DEFINITION: Under direction of a supervisory official, will provide high-quality healthcare services, wellness education and health promotion to the residents of the Housing Authority of the City of Passaic properties; with focus on improving health outcomes, promoting healthy lifestyles and ensuring residents have access to essential health services; will work collaboratively with local healthcare providers and community organizations to coordinate care resources for the residents.

GENERAL DUTIES:

  • Conducts health assessments and screenings (e.g. blood pressure, glucose monitoring) for residents.
  • Provides individualized health counseling and education on disease prevention, healthy management and self-management of chronic conditions.
  • Assists residents with managing medications, understanding treatment plans and accessing healthcare services.
  • Develops and implements wellness programs (e.g. smoking cessation, weight management, exercise programs) tailored to the resident population.
  • Coordinates with local health professionals and social services to ensure residents receive appropriate care and referrals.
  • Organizes and leads wellness workshops, seminars and health-related events for residents.
  • Monitors resident health concerns and identifies high-risk individuals or individuals with special needs who may need further interventions or specialized care.
  • Maintains accurate health records and documentation according to HIPAA guidelines.
  • Collaborates with property management, social workers and resident services staff to address health-related concerns within the community.
  • Participates in case management meetings and provides updates on resident health and wellness.
  • Stays updated on community resources and healthcare opportunities to benefit residents.
  • Other Housing Authority activities, as assigned.

QUALIFICATIONS PROFILE:

  • Registered Nurse (RN) license in the state of New Jersey.
  • A minimum of two (2) years of full-time clinical nursing experience, preferably in community health, public health or a similar setting.
  • Experience working with diverse populations, particularly low-income or underserved communities.
  • Strong communication and interpersonal skills, with the ability to build trust and rapport with the residents.
  • Demonstrated commitment to maintaining confidentiality and privacy of resident health information.
  • Bilingual (English/Spanish) preferred.

KNOWLEDGE AND ABILITIES:

  • Knowledge of chronic disease management, health promotion and wellness education.
  • Strong organizational and project management skills to plan and coordinate wellness programs and events.
  • Strong problem-solving skills to identify barriers to resident engagement and develop creative solutions.
  • Familiarity with wellness technology platforms and tools for tracking resident participation and outcomes.
  • Ability to prepare clear, sound, accurate and informative statistical and other reports containing findings, conclusions and recommendations.
  • Ability to work independently and collaboratively with a multidisciplinary team.
  • Ability to analyze, comprehend and interpret laws, rules, regulations and procedures and to apply them with reasonable consistency.
  • Ability to compose business correspondence.
  • Ability to supervise the maintenance of records and files.
  • Ability to utilize various types of electronic and/or manual recording and information systems used by the agency, office or related units.
  • Ability to read, write, speak, understand, and communicate in English sufficiently to perform duties of this position.

PHYSICAL REQUIREMENTS: Must be able to physically perform the basic life operational functions of stooping, kneeling, crouching, reaching, fingering, grasping, talking, hearing, and performing repetitive motions. Must be able to perform sedentary work, exerting up to 10 pounds occasionally. Must possess the visual acuity to prepare and analyze data and figures, and do extensive reading.
