Position Overview

A successful Licensed Vocational Nurse (LVN) must be passionate about healthcare and want to make a difference in the lives of others while acting as a mission-driven catalyst to help Livingston Community Health deliver the best quality of care and excellent service to our patients and their families.

The principle function of the Licensed Vocational Nurse is to provide safe patient care by assisting Clinic Providers with treatments, procedures and clerical duties as defined in the Licensed Vocational Nurse Scope of Practice. The Licensed Vocational Nurse will demonstrate competence in patient care for infants, children, adolescents, and adult patient population. The LVN will ensure the delivery of quality patient care and efficiency of patient flow.

Essential Functions, Duties, and Responsibilities

Per-Diem minimum requirement of 16 hours per month. Conducts pre-visit planning, including identification of all preventive and chronic care due. Identifies and document chief complaints in Electronic Health Record (EHR). Collects vital signs and document in the EHR: height, weight, blood pressure, pulse, O2 sat, temperature, pain level, last menstrual period, and any preventive exams or screenings. Engages in agenda setting with patient; document patient agenda in EHR (or on intake paperwork); solicit and list patient questions and issues for physician. Collects and document in EHR: allergies, medical history, social history, family history, social history/risk factors (e.g., smoking, exercise, alcohol use, etc.). Performs medication reconciliation, including noting for the PCP: any new or discontinued meds, meds not taken as directed, difficulties obtaining medications, and side effects. Reviews patient problem list with patient; advise clinician of any changes or updates that need to be made to problem list (e.g., resolved urinary tract infection). Assists provider with procedures, including but not limited to: colposcopy, Paps, I&D, toe nail removal, biopsy, and laceration repairs. Provides patient education, including post-visit physician recommendations and instructions; provide health coaching. Prints and provides patients with summary lists for review and confirmation (e.g., meds, problem list and allergies) as well as other documents requested by patient. Administers immunizations as directed. Administers medications as ordered by the physician, including but not limited to: nebulized medications for breathing treatments; injections Toradol (ketorolac), Depo-Provera, Rocephin (ceftriaxone), penicillin (PCN), Sublocade, Vivitrol, and other medications ordered. Collecting Samples for routine lab tests Calls new and renewed prescriptions to patients preferred pharmacies. Tracks lab orders through to receipt of results and patient notification (call or letter) document in the EHR. Monitors patient flow and assist staff to maintain efficient patient flow. Reviews and completes Unbilled Orders Report to ensure charge capture. Responds to all emergencies in the clinic. Responds to patient inquiries either by phone, in person, or by responding to a phone message and documents inquiry in the EHR. Processes Prior Authorization for Medications, Durable Medical Equipment and Medical Supplies. Conducts monthly site audits. Conducts weekly and monthly medication inventory and ordering (medications, vaccines and clinic supplies). Runs Point of Care (POC), CLIA waived, controls per protocol. Assures that all clinical protocols and nursing workflows, policies and procedures are followed. Attends periodic meetings (nursing, mandatory trainings, operations). Other duties and projects as assigned.

Education, Knowledge, Skills, and Abilities

Education

Possession of a valid CA license as a Vocational Nurse. Current BLS card (supported by the American Heart Association). Minimum one year experience as LVN in an outpatient clinic, preferred.

Knowledge, Skills, and Abilities -

Broad knowledge base of physical, mental, and social needs of our underserved population. Knowledge of common safety hazards, infection control, and precautions to establish a safe work environment. Consistently demonstrates safe and accurate technical skills. Skilled in identifying problems and recommending solutions. Able to effectively prepare and maintain electronic medical records, write reports, and respond to correspondence. Ability to react calmly and effectively in emergency situations. Ability to work with ethnically diverse populations. Ability to apply creative problem-solving skills to the complex issues that arise. Able to establish and maintain effective working relationships with patients, medical staff, staff members, and family caregivers in a pleasant, patient, and professional manner. Multi-language skills preferred.
