SUMMARY:



The Team Case Assistant coordinates patient services under the direction of the team manager/designee. This includes but not limited to prioritization and coordination of daily patient schedules, requesting additional services such as aide, therapy, social work, interpreter services, and ensuring that referrals for services are appropriately assigned in a timely fashion.



STATUS: Full Time



LOCATION: Home Care at Monroe Avenue



DEPARTMENT: Home Care Team Case Assist



SCHEDULE: Weekdays, 7:30am-4pm; flexibility required; Holiday rotation required



ATTRIBUTES:



  • Excellent interpersonal skills and customer service skills.

  • Ability to multi-task in a fast paced environment.

  • Proficient use of Microsoft office.

  • One year of experience in home care setting or medical office preferred.




RESPONSIBILITIES:




  • Customer Service. Manages multiple phone calls in a timely and professional manner providing appropriate follow up to assure callers' needs are met. Checks email in a timely manner providing follow up to assure sender's needs are attended appropriately. Provides exemplary customer service.




  • Team Support. Provides field staff assistance and resources for appropriate day to day issues as they arise under the direction of the team manager/designee. Builds and maintains strong, communicative relationships with the team. Performs general office duties/projects including but not limited answering phones, documentation/order processing, copying, filing, mailing, and obtaining/ordering office supplies. Assists in orientation of new staff, role modeling, participates in quality assurance activities. Attends team meetings and other internal meetings as requested.




  • Organization and Task Management. Collects, organizes and reviews various programs for information and reporting and initiates follow up. Updates the EMR per agency standards. Performs functions related to transfers or discharges as required by regulation.


PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.



PAY RANGE: $17.50 - $19.40

The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
