Job Description:

Primary Function:
The Certified Nursing Assistant performs various patient care activities and related nonprofessional services necessary in caring for the personal needs and comfort of patients under the direction and guidance of professional nursing personnel.
Training:
Orientation to Hospice, Home Health, Inpatient and Palliative Care services and any other training that may be necessary to complete the assigned duties.
Supervision: HHH Nurse Manager
Qualifications:
High school diploma preferred. Completion of certificate program in nursing care from an accredited training program with current listing as CNA-I with the North Carolina Department of Health and Human Services Regulation.Six-months to one year of experience preferred.On-the-job training to become familiar with Hospice policies, procedures, and routines.Ability to work with the team concept of patient/family care outside a hospital setting.Ability to work interdependently.Commitment to the Hospice concept and standards.Must be physically capable of providing patient care in various institutional settings to

Must be current in health immunization requirements of North Carolina and Hospice of Davidson County.Must be able to interact with patients, family members, visitors, co-workers, and medical staff under a variety of circumstances.Regular, reliable attendance is essential.CPR trained/certified.
Job Duties:
Adheres to written and oral instructions for patient care per the nurse.Performs specific direct patient care duties of a CNA I as outlined by the Nurse Aide Registry Act and assigned by Hospice Licensed Nurse.Aware of basic pain management including use of pain rating scales. Recognizes and communicates with the Hospice nurse the need for intervention regarding pain when indicated.Contributes to the maintenance of nursing care plan.Reports all unusual conditions or reactions to the patients primary nurse and/or HHH Nurse Manager.Completes documentation of patient care and treatment per Hospice of Davidson County Policy.Helps keep supplies organized. Places charge slips in patient notebook when removing supplies to maintain inventory of stock.Attends weekly team conference, meetings and educational programs as necessary.Maintains a cooperative and collaborative relationship with all Hospice personnel that facilitates a positive practice environment and interdisciplinary philosophy of services to families.Understands and adheres to the mission and advocates its philosophy.Maintains confidentiality of Protected Health Information (PHI) at all times.Acknowledges importance of team awareness.Addresses patients, co-workers, and other healthcare professionals in a pleasant and respectful manner.Conveys instructions, directions, and information to co-workers.Consults with immediate supervisor as issues and concerns arise while maintaining consistent and concise lines of authority.Performs housekeeping services that are necessary for the health and safety of the patient.Cleans toys in playroom each shift and as needed.Follows the guidelines for universal precautions when providing patient care.Assists in answering the telephone and identifies faculty and self. Directs call to appropriate person or assists caller as needed. Takes messages, documents the date, time, name of caller, phone number and message when applicable.Answers patients call light timely.Adheres to all policies, procedures and regulations regarding patient care, conduct, safety, fire, security and risk management.Consistently represents Hospice of Davidson County to all internal and external customers with integrity and professionalism.Attends required in-services on an annual basis:Risk ManagementBloodborne Pathogens/TBBody MechanicsFire SafetyGeneral Safety/Hazardous MaterialsPatient/Staff RelationsMaintains a safe and clean work area to promote workplace safety.Clocks in/out on computer.Documents services and expenditures for reimbursement purposes.Performs other duties or special projects as requested by the HHH Nurse Manager.Participates in hospice sponsored in-service training.Participates in QAPI program as needed.Participates on Hospice committees as needed.Performs job duties in accordance with all Hospice policies, procedures, and professional and community standards.Abides by the Hospice Professional Code of Conduct.

The noise level in the work environment is usually moderate.
Status and Hours of Work:
This position is classified as non-exempt, PRN. Usual working hours are 12-hour shifts. The facility is in operation 24 hours a day, seven days a week. This position is eligible for overtime pay.
