Under the direction of the Director of Hospice & Palliative Care and within established organizational and departmental policies and procedures, responsible for providing social casework to clients and families dealing with terminal illness, having difficulty in social/emotional functioning, primarily as it adversely affects the stability of the client's medical condition or the client's response to a prescribed medical regimen. Refer and aid in the referral process to social agencies. Assist the Interdisciplinary Team (IDT) in understanding the significant social and emotional factors to the client's health problem.
Qualifications
Master's degree from a school of social work accredited by the Council on Social Work Education required.
LICSW preferred.
Minimum of one (1) year experience in a health care setting required.
Current licensure by the Board of Registration in Social Work required.
Demonstrated strong verbal and written communication skills and good interpersonal skills required.
Demonstrated knowledge of the principles and practices of social work and of resources. Available in the community required.
Current driver's license, good driving record and access to or own reliable transportation required.
