Job Title: Hospice RN Case Manager



Duties & Responsibilities: Hospice RN Case Manager sought for 8 week assignment, with the potential to extend. Schedule would be 5x8 days, Monday-Friday. Mileage is reimbursed at $0.50 per mile.



Minimum Requirements:

• IN or Compact RN license


• 1+ year hospice in home settings experience within the past 3 years


• BLS


• HCHB experience preferred but not required
