POSITION: Recovery Adult Care Coordinator (ACC)
EMPLOYMENT STATUS: Full-Time (FT)
POSITION SUMMARY:
Bridgeway Center, Inc. (BCI) is seeking applicants that want to work with clients to effectively address clients' care by assessing needs,
developing, monitoring, and facilitating interdisciplinary approaches. This position is responsible for managing an assigned caseload of
clients having mental illness diagnoses and for assessing client needs, developing, implementing, and reviewing service plans, and working
with other community resources in meeting/achieving client service needs.

The Care Coordination Program (CCP) provides services that help individuals link to needed medical, social, educational, psychiatric,
psychological, and other services. This includes direct assistance in accessing services, Coordination of Care (CoC), oversight of the entire
case, and linkage to appropriate services. Care Coordination includes (1) assessment of the eligible client to determine service needs; (2)
development of an individualized integrated service plan; (3) referral and related activities to help the client obtain needed services; (4)
monitoring and follow-up; and (5) evaluation. The CCP department educates the persons served and their support system regarding
diagnosis, treatment, and recovery. This position will work with clients in the Judicial system.

This position is funded by revenue appropriated by contract and your employment is contingent upon continued funding
of this position.

Applicants must be outgoing and enjoy working with the public, professional, and para-professional team. Have the ability to collaborate
and communicate with BCI departments and external stakeholders. Must be able to multi-task and always remain professional to ensure
that each client has a positive and helpful therapeutic experience.
