The Case Manager will have the responsibility of accomplishing the case management functions of the Adolescent Treatment Program as described in the Adolescent Treatment Program Plans.
Daily Operations
Ensure that clients are sufficiently monitored and supervised at all times during work shift.
Maintain clinical records and required paperwork in compliance with agency standards.
Assist in the collection and reporting of statistical data to funding and other agencies as required.
Maintain caseloads as assigned by the Program Manager
Document Office and Community based case management notes daily/ weekly
Complete Weekly fees sheets
Drug screen clients daily/weekly
Complete Weekly Progress Notes and perform Case Reviews weekly
Identify and interview eligible clients (intake), and coordinate substance abuse intervention.
Provide treatment planning and access client to appropriate service providers (referral).
Coordinate, facilitate, and plan for weekly client groups presentations
Acquire and maintain a functional knowledge of internal and funding agency standards of service delivery and documentation.
Clients Discharge Summaries
Attend weekly Treatment team meeting and do case presentations
Acquire, maintain, and practice skills regarding sensitivity to the cultural and socioeconomic backgrounds (including sexual preferences) of clients and others contacted in carrying out job responsibilities.
Create clients Treatment plans
Adhere to the guidelines as stated in the Meharry Medical College Policy and Procedures Manual.
Attend all appropriate client care conferences and staff meetings.
Be courteous, tactful, and discrete in relating to the public, other staff, and clients.
Participate in agency training activities as required by supervisor, training coordinator, and funding agency.
Respect and protect client's rights (including confidentiality) and ensure that client's respect the rights of others by appropriately intervening and/or reporting when program rule violations occur.
Provide substance abuse and family education seminars.
Deliver prevention/education seminars to the community.
Participate in health fairs and other outreach activities on weekends and evenings as required.
Transport clients to, from center, and on excursions.
Maintain F-endorsement and licensure requirements in accordance with established guidelines.
Provide weekly clients updates to Program manager
Acquire and maintain a functional knowledge of internal and funding agency standards of service delivery and documentation.
Provide back up for other case management staff in the event of illness, vacations, or other necessary absences from work.
Performs other related duties as assigned by Program Manager.
Required Skills

Required Education and Experience
Bachelor's degree (B.S. or B.A.) from a four-year college or university, or a minimum of two years related experience and/or training, or equivalent combination of education and experience.
Must obtain and maintain a valid Tennessee For-Hire (F endorsement). Driving record cannot contain items which prohibit or inflate cost of insuring driver.
Preferred to be self-identified as being in recovery from mental illness, substance abuse, or co-occurring disorders.
