SCOPE OF ROLE

Reporting to the Medical Director, Primary Care, the Nurse Practitioner Float, Primary Care is primarily responsible for managing walk-in patients and ensuring patient care continuity by floating to other sites as needed for coverage. The Nurse Practitioner Float, Primary Care maintains seamless care delivery during the absences of regular providers due to vacations, illness, or professional development activities. This role works collaboratively with other clinical staff to facilitate a cross-discipline team approach to medical care.

HOW YOU'LL SUCCEED


  • Efficiently handle walk-in visits, providing immediate assessment, diagnosis, and treatment plans for acute and chronic health issues.

  • Serve as a floating nurse practitioner across multiple sites, stepping in to provide continuity of care when regular providers are absent.

  • Be readily available to cover shifts across locations, often at short notice, to ensure uninterrupted patient services.

  • Adapt to varying schedules and locations, including evening hours, maintaining high standard of care across different environments.

  • Work closely with other health care professionals across various sites and school based settings to ensure integrated, comprehensive care delivery.

  • Demonstrate high clinical skills particularly in acute care settings, with the ability to make swift, sound clinical judgments.

  • Show flexibility in adapting to different clinical environments, visit modalities (telehealth vs in-person), and patient needs.

  • Maintain clear and effective communication, both within healthcare teams and with patients, adapting messages to different contexts and sites.

  • Demonstrate proficiency in using the EHR system to efficiently manage patient information and care documentation.

  • Obtaining and legibly recording a detailed medical history of each patient, including, but not limited to, the patient's chief complaint, history of present illness, and past medical, social, and family history, all as medically appropriate.

  • Demonstrated clinical competence including physical assessment, health teaching and counseling, and make sound clinical judgments based on assessment of the physical, psychological, social and environmental needs of the client.

  • Prescribe treatment for illnesses within the Nurse Practitioner's scope of practice, based on standing orders and nurse practitioner formulary.

  • Determine which problems are within scope of practice, and which require referral to a physician according to mutually accepted guidelines.

  • Analyzing reports and/or findings of tests and personal examinations to assess the medical condition of the patient, including diagnosis and prognosis.

  • Referring the patient to other services within Signature Health, other physicians, including specialists and other health care providers, when and if needed.

  • Prescribing medication and treatment for the patient that is consistent with standard clinical practice and the APRN's education, training, and scope of practice.

  • Performing such clinical procedures and therapeutic techniques as are consistent with APRN's prior practice patterns and training.

  • Promoting the professional practice of Signature Health.

  • Timely reporting of major unusual incidents and other information required to appropriate governmental authorities and as required for Signature Health reporting.

  • Participate in Signature Health quality assurance and peer review activities.

  • Performing other duties assigned by Signature Health from time to time that are commensurate with professional services normally and customarily performed by an APRN.

  • In providing primary care services, Nurse Practitioner Float, Primary Care will exercise independent medical judgment consistent with approved methods and practices in his/her specialty and in the best interest of the patients. All other activities are to be conducted with the consultation and assistance of the Medical Director, Primary Care. At all appropriate times, treatment activities should be performed as a team function.

Requirements

KNOWLEDGE & EXPERIENCE


  • Ohio Licensed Advance Practice Nurse.

  • Graduation from an accredited university with a background in science, including a BA or BS in Biology, Chemistry, Nursing, Anatomy and Physiology, Public Health, Behavioral Science, or a similar degree.

  • Current DEA registration.

  • American Heart Association (AHA) Basic Life Support (BLS) certification required

  • Minimum of 5 years of clinical experience, preferably within a Federally Qualified Health Center.

  • Proficient in Microsoft Office and Electronic Health Records


WORKING CONDITIONS

  • Requires some exposure to physical risk.

  • Minimal travel, work is normally performed in a typical interior/office work environment.

  • Hearing: Adequate to perform job duties in person and over the telephone.

  • Speaking: Must be able to communicate clearly to patients in person and over the telephone.

  • Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens.

  • Other: Requires occasional lifting and carrying items weighing up to 20 pounds unassisted.

  • Requires frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
