Job Details:
Title: LPN Case Manager
Location: Kissimmee, FL
Type of Engagement: Contract
Rate: Competitive and Dependent on Experience
Specific Duties:
1. Discharge Planning
2. Provide a point of communication between specialist. clinician and loved ones.
3. Arrange referrals or recommend patient specialist where needed
4. Evaluate the progress and recovery of the patients recovery after discharge and seen in the office.
5. Ensuring that patient records are thoroughly and accurately updated.
Must have qualifications/experience:
1. Time management skills
2. Great commutation skills
3. Chart review experience
4. Ability to document clearly and accurately
5. Be a team player
Hours: 8:00 AM - 5:00 PM (Monday - Friday)
