Community Integration Facilitator- Columbia location- at HUMANIM INC in Baltimore, Maryland

Job Description:

Reporting to the Manager of DD-SE Program, the Community Integration Facilitator is responsible for facilitating and actively participating in Person Centered Planning for adults with Intellectual/Development Disabilities while collaborating with team members to provide clients with meaningful workshops/modules on a variety of topics including but not limited to life skills, social skills training, and vocational and recreational activities. Setting priorities and objectives to align with treatment goals. This role will require some transportation of clients via personal or company vehicles.
